Responsibilities Job Summary The DIETITIAN evaluates nutrition risk criteria, assesses nutritional status and develops/implements plan of nutrition care based on evidence based practice and diagnosis related care guidelines. Reviews integrated nutrition screening reports and accurately identify patients at nutritional risk. Completes nutritional assessment using established evidenced based, diagnosis related nutrition care guidelines. Evaluates patient outcome and progress toward achieving nutrition goals. Revises intervention and/or education plan. Serves as nutrition specialist and resource to physicians, HH departments, allied health professionals, and accredited nutrition programs. Prepares written outcome oriented documentation for the patient’s medical record on all consultation in accordance with evidence based practice guidelines. Researches, develops and implements patient nutrition care protocols and educational materials, based on evidence based practice. Participates in performance integration and improvement of patient care processes and department or system process improvement initiatives. Assesses nutrition education needs and provides individualized education to patients and families. Adapts to changes in work environment. Recognizes and performs duties and responsibilities which need to be done although not specifically assigned. Qualifications Education Master's Degree as required by the Commission on Dietetic Registration (effective Jan. 1, 2024) or bachelor’s degree grandfathered prior to this update Required Experience None Required Licenses and Certifications Food Service\RD - Registered Dietitian Current Registration by the Commission on Dietetics Registration Upon Hire Required
