POSITION SUMMARY:

The Assistant Director of Health Services (ADHS) coordinates the provisions of Hospice services for each patient and family, coordinates clinical services for the Hospice agency, supervises the interdisciplinary team members, and provides outreach to the community.

KEY RESPONSIBILITIES:

  1. Primarily responsible for taking and coordinating referrals and admission through the established organizations policies and guidelines.

  2. Coordinates Hospice services for each patient and family.

  3. Responsible for coordinating and training of personnel to nursing policies and procedures.

  4. Coordinates admission processes through assignments to clinical staff and/or performing field admission visits.

  5. Coordinates admission paperwork to ensure timely completion and accuracy.

  6. Makes field visits to evaluate appropriateness, determine eligibility for services and present Hospice program.

  7. Assigns clinical staff to patients and families based on the patient needs, including Chaplain, Social Workers, Volunteers, Dietary Counselors, therapists for PT/ST/OT, and bereavement coordinators.

  8. Implementation of employee relations practices necessary to establish a positive partner – organization relationship and promote a high level of partner morale.

  9. Follows all company policies and procedures, state and federal laws and regulations and charged with reporting violations to the appropriate supervisor.

  10. Ability to respond to change productively and handle additional projects as assigned.

  11. Communicates assessment findings to Attending Physician and initiates treatment plan per physician’s orders.

  12. Orders and approves DME and nursing supplies.

  13. Assists in provision of training on Hospice issues to clinical staff, patients and families, healthcare centers, nursing facilities, acute care facilities and community groups.

  14. Performs routine chart audits to ensure timeliness and accuracy of documentation and MD orders.

  15. Identifies, completes and follows up on quality assurance performance improvement reports including monitoring of infections and Opportunity Occurrence Program Reports.

  16. Facilitates team meetings and care plan review.

  17. Schedules on-call RN duties and provides backup coverage and administrative on-call duties.

  18. Communicates with Attending Physicians and Medical Director on routine basis as indicated.

  19. Completes competency assessments for nursing, certified nursing assistant, and aide partners.

  5. Ability to work effectively, independently and within an interdisciplinary team.

  6. Participate in center/agency surveys (Licensure/JCAHO) and any subsequently required reports.

  7. Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification as required.

  8. Attend and participate in mandatory inservices.

  9. Honor patients/residents’ rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.

  10. Comply with corporate compliance program.

  11. Report job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.

  12. Follow established safety regulations, to include fire protection and prevention, smoking regulations, infection control, etc.

  13. Follow established safety procedures when performing tasks and/or working with equipment.

  14. Perform other related duties as necessary and as directed by supervisor.

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

  • Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice
