Position Overview:
Provides diabetes self-management education/training (DSMT) and/or medical nutrition therapy (MNT) to individuals, family members, and groups. Manages the care of a distinct patient population through assessment of educational needs, nutritional guidelines, lifestyle interventional goals, and counseling. Coordinates patient care in collaboration with other healthcare providers (physicians, pharmacists, case managers, social workers, etc.) to develop effective individualized protocols (medication, therapy, nutrition, devices, etc.) that empower patients with the techniques and behavior modifications to help control their disease, avoid potential complications, and bring about lasting improvements in their quality of life.

JOB ACCOUNTABILITIES:
DIABETES EDUCATION.
Performs assessments based on laboratory values, physician instructions, medications, medical history, and patient’s lifestyle factors.
Develops nutritional and educational care plans based on patient assessment, learning needs, preference and capabilities to build self-care behaviors to manage diabetes.
Delivers diabetes self-management education, DSMT and medical nutrition therapy (MNT) individually and in groups.
Consults on basic insulin initiation, and use.
Evaluates and triage all referrals/patient phone calls and initiate interventions, which may include interdepartmental collaboration (staff, physicians, and other healthcare providers) to provide quality patient care.
Maintain appropriate and timely communication (through EPIC, online applications, email, and phone) with patients, medical staff, and other departments to achieve patient care objectives.
Participates in case conferences, diabetes, and clinic meetings, as scheduled.
CONTINUOUS IMPROVEMENT.
Assists in the development and revision of guidelines, procedures, and protocols related to the development of patient care.
Works collaboratively to develop new programs, services and patient educational materials as needed.
Collects, analyzes and interprets program data.
Participates in performance improvement initiatives related to program.
COMPLIANCE.
Provide initial and ongoing patient education in individual and/or group formats according to the scope of practice, evidenced-based practice guidelines, and the American Association of Diabetes Educators (AADE) Code of Ethics for diabetes educators to manage diabetes.
Ensure documentation of care is timely and accurate, and meets requirements set forth by credentialing entities and for insurance reimbursement.
Maintains strictest confidence of all patient protected health information (PHI) and protects all PHI from accidental, intentional, or inappropriate disclosure.
Build collaborative relationships with peers, other healthcare providers, and vendors to achieve departmental and corporate objectives.
Ensure the privacy of each patient’s protected health information (PHI).
Adheres to department policies and procedures, the Academy of Nutrition and Dietetics/Commission on Dietetic Registration Code of Ethics and the Academy of Nutrition and Dietetics Scope and Standards of Practice.
EDUCATION:
Bachelor's Degree
CERTIFICATION & LICENSURE:
BLS-Basic Life Support Healthcare Provider
SKILLS AND KNOWLEDGE :
Proven basic competency in standard concepts, practices, and procedures within the diabetes education specialty field that demonstrates clinical judgment, technical skills, and knowledge base.
Demonstrated basic knowledge of the use of diabetes monitoring and managing equipment, including blood glucose monitors, insulin injection devices, and the computer software associated with these devices.
Skilled in Motivational Interviewing and basic behavioral counseling skills.
Demonstrated strong group facilitation skills.
Persons in this position will be expected to gather the required hours to become eligible for and sit for the CDCES exam within 2 years of hire. Strong written and verbal communication skills to communicate with a diverse group of patients, care givers, colleagues, providers and specialists.
Compassionately and effectively interact with patients and staff with diverse ages, backgrounds, cultures, values, beliefs, and behaviors.
Able to work collaboratively in diverse teams.
Knowledge of adult learning theory and ability to provide individual and group education on a variety of topics related to diabetes and other chronic conditions.
Attention to detail with time management and organization skills.
Solid knowledge of computer applications, such as Microsoft Office Suite and EPIC to enter/update electronic medical records (EMR).
Prioritize activities and work within defined policies, procedures, and practices to achieve objectives and meet deadlines; help determine appropriate approach for new assignments.
Work independently as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.
Communicate technical or sensitive information clearly and professionally to diverse audiences (patients, patients’ family/support group, physicians, and staff).
Capable of performing routine nutritional therapy as well as a basic understanding of human anatomy, physiology, pathology, and medical terminology.
Identify, evaluate, and resolve standard problems by selecting appropriate solutions from established options.
Treat patients ranging in age from social age to geriatric.

Pay Range is $38.03 to $53.24 / hour
The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
