This position is grant funded and part of the WellMed Charitable Foundation. The goal of this job is to assist in protecting, maintaining and building trust between patients, their local community, and the clinical team by understanding patients' needs and capacities; facilitating patients' compliance with clinical treatments; supporting patients as they implement changes that improve their self-care; linking the patients to community based services to improve their living situation, and educating providers about the contextual social determinants of their patient's health.

Primary Responsibilities:

  • Performs all duties within the scope of a Community Health Worker’s practice: Community Health Workers (CHWs) are frontline public health workers who are trusted members of and/or have an unusually close understanding of the community served. This trusting relationship enables CHWs to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. CHWs also build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy

  • Receives assignment of patients by supervisor and promptly arranges face to face encounters at agreed upon site to begin trusting relationship

  • Develops ongoing relationships with patients and members of the patients' trust network to help the implementation of changes that improve patients’ self-care

  • Utilizes documentation of work activities to help patients and their families evaluate and learn from actions taken and plan new actions to advance patient’s self-care

  • Reports weekly to supervisor to account for all activities and participate creatively in continuity of patient’s care

  • Maintains ongoing communication with clinical team as needed to facilitate patients' clinical care and compliance with treatments

  • Informs clinicians of patients' overall self-care status and contextual issues as appropriate

  • Maintains and models self-care and practices safety protocols as instructed, relying on supervisor and coworkers for help and advice

  • Is proactive and creative in asking for and responding to co-worker’s advice and support in the field

  • Participates in specialized training in working with someone with dementia

  • Attends all required group meetings with supervisor and co-workers to carry out and/or improve work

  • Participates in neighborhood and public activities that foster family and community health as required. Performs other community health duties as assigned

Required Qualifications:

  • High School Diploma or GED equivalent

  • Current certification as a Community Health Worker with a program accredited by the State of Texas, or ability to attain certification within 180 days

  • Basic computer literacy with Microsoft Office

  • Proven good communication and customer services skills

  • This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease

Preferred Qualifications:

  • Experience in working with older adults

  • Knowledge of local community services and non-profit organizations

Physical & Mental Requirements:

  • Ability to push or pull heavy objects using pounds of force

  • Ability to stand for extended periods of time
