EO/AAPosition Overview: The Intensive Outpatient Clinic (IOC) at University of Utah Health is seeking a qualified Nurse Practioner to be an integral part of an interdisciplinary team of medical and behavioral health professionals delivering exceptional patient-centered care for a targeted population of individuals with complex medical and psychosocial needs. Providing comprehensive and integrated care, the IOC aims to improve clinical outcomes and decrease the overall cost of care for the population we serve. In 2023, Governor Coxs One Utah Health Collaborative recognized the IOC has as leader in novel comprehensive care delivery and sustainable financial innovation. The IOC was also featured in Meet Me Where I Am a short film that premiered in the 2022 Sundance Film Festival that tells the story of one IOC patient and how this clinic provides unique integrated care.This position provides both primary and specialty medical services in a University Health clinic or hospital unit and works collaboratively with the medical and behavioral health teams to deliver comprehensive care oriented around treating the bio-psycho-social needs of the IOC patient population. Clinical activities include, but are not limited to: Ambulatory Office Visits with History & Physical Exam review, Telehealth Visits, Supporting Care Coordination, and Medication Management. The ideal candidate will also have a working knowledge of trauma and substance use disorders.The IOC exclusively serves a Medicaid population and operates under a capitated model with the University of Utah Health Plans. This position reports to the IOC Medical Director and will support the IOC patient population and the clinic team located at the University of Utah Population Health Center near Rose Park.Corporate Overview: The University of Utah is a Level 1 Trauma Center and is nationally ranked and recognized for our academic research, quality standards and overall patient experience.
