Community Health Worker I

LOCATION: West Salem Clinic

JOB STATUS: Full-Time, 40 hours per week, Monday – Friday with occasional Saturday & after-hours availability for community events

SIGN-ON BONUS: $1,000 (Internal candidates that are currently employed here at NWHS are not eligible for a sign-on bonus.)

REFERRAL BONUS: $500 (Referral bonus’ expire at the close of this job posting.)

YOUR ROLE:

As a Tier 1 Community Health Worker (CHW-I), you will focus on addressing the social determinants of health and barriers that our patient and client faces, with the expectation to demonstrate respect towards diverse socioeconomic communities, cultural humility, commitment to empowering others, continuous learning mindset, trust-building capabilities, innovation, persistence, reliability, and resilience in handling stressful situations. Additionally, you are expected to offer resources and support while empowering clients to manage their own health and engage in training in Trauma-Informed Care, Motivational Interviewing, and Community Health Work. A CHW-I must demonstrate the ability to adapt services based on individual client and natural support system needs.

SPECIFIC DUTIES:

  • Builds trust and rapport with clients through professional communication.

  • Utilizes motivational interviewing skills to offer patient-centered resources.

  • Documents screening forms and interventions in NextGen.

  • Responds to referrals and tasks within specified timeframes.

  • Works within an interdisciplinary care team.

    QUALIFICATIONS:

  • Valid Oregon Driver's License and proof of automobile insurance.

  • Must be able to build effective working relationships, solve problems, direct patients to resources and manage information. Understand your role’s boundaries and be able to clarify them to your patients so you can maintain a professional relationship.

  • Excellent phone and computer skills required.

  • Must be able to find creative solutions to patient barriers that require initiative and creative thinking to locate the appropriate resource.

  • Ability to work independently, providing closed looped follow-up to the care team and community partners.

  • Ability to prioritize time and resources, based on multiple factors, including risk level and readiness for change.
