Opportunity Overview

Under the direct supervision of the Nursing l Director of Addiction and Recovery Services, the Clinical Outreach and Recovery RNCM, in collaboration with the Medical Director of Integration and Innovation, will provide continuing and comprehensive health care for our most vulnerable populations. Utilizing a harm reduction approach, the Clinical Outreach and Recovery RNCM provides clinical leadership and skilled nursing services, including triage, assessment, case management, education, and outreach. The role involves ongoing assessment, intakes, evaluations, teaching, initiations, stabilizations, and monitoring of patients.

Responsibilities:

  • Uses a patient-centered approach with attention to non-stigmatizing language while implementing care plans

  • Uses fundamentals of harm reduction approaches such as overdose prevention and education, Narcan training, fentanyl test strips, and low-barrier care

  • Uses a trauma-informed care approach

  • Conducts initial assessments and intakes, gathering histories including social, medical, mental health, substance use, and medication history.

  • Provides direct and comprehensive care at shelter locations in the NSCH service area under the direction of the Director of Integration and Innovation.

  • Coordinates clinical outreach efforts, including “pop-up clinics” and portable vaccine clinics

  • In collaboration with Direct of Integration and innovation and Clinical Outreach Specialist, identifies and develops opportunities to engage with vulnerable populations within and outside the health centers.

  • Provides longitudinal integrated care of patients on the Clinical Outreach Panel in addition to time spent at the shelter

  • Triages phone and walk-inquiries for Recovery Services

  • Review assessments with the SUD clinical and primary care teams to determine appropriate care plans.

  • Collaborates and communicates with the SUD team and meetings as required.

  • Provides ongoing education, support, and service coordination for patients.

  • Collaborates with the SUD treatment support team, which consists of SUD providers, SUD navigators, recovery coaches, and the BH team.

  • Serves as a liaison and provides education and support for primary care teams and community partners such as hospitals, VNA, labs, Correctional Facilities, or other medical offices, outreach, and teams

  • Ensures meticulous and timely documentation.

  • Reviews commonly found results with patients per protocol.

  • Administers immunizations and other medications as providers request or per the protocol and accurately documents them in the EMR.

  • Coordinates with the pharmacy and SUD / Clinical Outreach team on medication administration, prescriptions, and refills.

  • Counsels patients on relapse prevention and pharmacotherapy and provides referrals and resources for non-pharmacotherapy as indicated.

  • Provides follow-up care, scheduling clinical visits and telehealth visits

  • Engages in primary care and addiction-related projects.

  • Assists with phone triage, prescription refills, follow-up test results, and managing medical issues.

  • Assists providers in providing care: procedures, injections, EKGs, referrals, prior authorization, specialty pharmacy, security, and documentation of controlled substances.

  • Ensures appropriate storage and documentation of Controlled Substances and record keeping per protocol in the Controlled Substance Log

  • Assists with transitions of care

  • Assists in discharge planning for patients returning to outpatient treatment in collaboration with SUD Navigators and Clinical Outreach Specialist

  • Provides emergency treatment consistent with established policies and procedures.

  • Attends scheduled meetings.

  • Participates in health promotion and disease prevention activities, including screening, immunization, and health education events sponsored by NSCH.

  • Other duties as assigned

Requirements

  • Bachelor's Degree in Nursing

  • Licensure to practice nursing in Massachusetts

  • Background in caring for patients with substance use disorders preferred

  • Preferred comfort in working outside of traditional clinical spaces with vulnerable patient populations

  • Ability to travel between Health Center and community sites

  • Understanding of trauma-informed care and harm reduction

  • Certified as a registered nurse with a minimum of 3 years of experience in community health.

  • Preference will be given to bilingual/bicultural individuals who speak Spanish or Portuguese.

  • Ability to work effectively both independently and as part of a team

  • Strong communication and interpersonal skills, with a commitment to establishing a safe and respectful environment for team members

  • Ability to effectively work in a culturally diverse environment and with marginalized populations such as homeless and transgender individuals and those with substance use disorders

  • High level of commitment to continuous improvement in the quest for comprehensive and quality healthcare

This is an administrative job by nature but requires mobility and the ability to commute from various offices and patient homes. Must be able to use a computer/laptop and phone in addition to the mobility.

As a health center, NSCH ensures that all employees comply with the annual requirements for vaccinations and health screenings.
