Missouri Based Telehealth Pharmacist
Description
The Telehealth Pharmacist will provide high touch tele-health services which include, but are not limited to Medication Therapy Management (MTM), basic medication education, referrals to support services, etc.
• Counseling participants on the importance of medication adherence
• Providing medication education
• Medication therapy recommendations
• Connecting participants with our community-based resources as needed.
• Assessing a participant’s health status
• Developing a medication treatment plan
• Program documentation - proper documentation of each service provided to patients including: treatment, evaluation, furthering testing, findings, results and prescriptions
• Coordinate with patient to provide regular touchpoints
Additional Skills & Qualifications
Pharmacist
Medication Management experience
Schedule flexibility M-F, some weekends
Must Reside around the St. Louis Missouri area. Must have a Pharmacy license from the Missouri Board of Pharmacy
- Role will be directly supporting patients in the St. Louis metropolitan area.
Role will be a longstanding contract with extension.
Pay and Benefits
The pay range for this position is $50.00 - $55.00
