Job Overview:

In an out-patient setting, the Advanced Practice Nurse (APN) provides client-focused, evidence-based care to diverse individuals and their support systems, to maximize health, well-being, functionality, and resiliency. The APN provides a process of patient care that includes assessment, diagnosis, treatment planning, intervention, and evaluation of outcomes. The APN may routinely collaborate with other health care providers, internally and externally, as well as community and social service organizations. The APN is guided by professional standards and demonstrates clinical competencies developed through graduate education and certified by the ANCC. The APN adheres to the Scope of Practice defined by the NJ Board of Nursing (NJSA 13:37). The APN continually seeks opportunities to develop further as a clinician and an advanced practice professional.

Qualification-

+ + Masters and/or DNP degree with PMHNP concentration and board certified by the American Nurses Credentialing Center (ANCC).

  • Current State of New Jersey APN license, CDS, DEA and BLS, or ability to obtain prior to starting employment.

  • Three to five years of relevant work experience preferred.

  • A track record of success working in an environment of collaboration, transparency, mutual respect, and shared passion for excellence and innovation.

  • The ability to express oneself clearly and concisely both orally and in writing.

  • The ability to plan, organize, prioritize daily tasks and work with minimal supervision.

Scheduling Requirements:

  • Full time, 40 hours per week, days.

  • Hybrid opportunities may be available based on the needs of patients and the organization after successful completion of orientation.

Essential Functions:

  • Provide a process of patient care that includes assessment, diagnosis, treatment planning, intervention, and evaluation of outcomes related to the presenting problem.

  • Engage with other professionals to assess, plan, deliver, integrate, and evaluate care.

  • Support, deliver, and/or coordinate appropriate level of care based on patient acuity.

  • Conduct psychiatric evaluations, SBIRT and motivational interviews; utilize diagnostic criteria based the DSM-5-TR.

  • Educate patients and families about treatment plans, interventions, medications, mental illness, symptom relief, comorbidities, and strategies to enhance overall health.

  • Provide medication management based on target symptoms, psychopharmacological evidence, Federal & State Regulations.

  • Complete written evaluations, medical records, progress notes and service coding utilizing an EMR system (familiarity with Epic is a plus)

  • Order and review appropriate laboratory and diagnostic procedures.

  • Provide brief psychotherapy utilizing principles of CBT, DBT, psychodynamic and/or supportive therapy as needed.

  • Maintain confidentiality of patient treatment and records in accordance with HIPAA.
