What you'll do: Provide patient care to residents, Comply with state and federal regulation, Work collaboratively with members of the healthcare team, Serve as a primary coordinator of all disciplines for well-coordinated patient care, Assess and coordinate patient's discharge planning needs with members of the healthcare team. Requirements: Valid state license(s), Excellent verbal communication skills and strong interpersonal skills, High integrity and well-grounded work ethic, Agility and flexibility with changing priorities, Team-oriented and a practical team-player, Professional demeanor, At least one year experience providing care and services in long-term care (preferred).
