University of Rochester Supervisor, Social Worker in Rochester, New York

GENERAL PURPOSE:

The senior social worker collaborates with the Clinic Director in the ongoing management and operations of Older Adult Services. They serve as a member of the Leadership Team, participating in the development, implementation, and ongoing evaluation of initiatives to effect integrated, collaborative, patient-centered, recovery orientated care. The senior social worker is also responsible for the clinical and administrative supervision of assigned social workers and CCBHC positions. The senior social worker is also responsible for providing clinical services, psychosocial assessment, psychiatric diagnosis, and treatment for older adult patients 65 years of age and older. The senior social worker functions as primary therapist and is responsible for the overall coordination and implementation of the treatment plan for patients assigned to their caseload. They participate on an interdisciplinary treatment team and develops liaisons with health and social agencies in the community, as well as with other health professionals within the hospital to support evaluation and treatment of the patient. The senior social worker also provides individual group treatment, and crisis intervention as appropriate. The senior social worker may provide family treatment and case management services as indicated.

The senior social worker may also collaborate in policy and program development, operations managing, and plan program activities for the program area.

The senior social works also acts as the Behavioral Health Module Super User and Education Coordinator.

RESPONSIBILITIES:

  • As a member of the interdisciplinary leadership team for the Older Adult Services clinic, collaborates to develop and implement policies, procedures, and programs.Participates in the development and/or execution of quality improvement and outcome measurement initiatives in the service area. Provides administrative and clinical supervision to therapists consistent with the Clinic and Department standards. Collaborates with program director to ensure professional development of staff.Participates in the development, implementation, leadership, and supervision of groups within the Older Adult Services.

  • Functions as a clinician to program patients.Provides screening, evaluation and group treatment. Performs psychosocial assessment and diagnostic formulations for patients and, when indicated, their families. Functions collaboratively and consultatively as member of interdisciplinary team. Coordinates with other resources in the community and within SMH to ensure continuity of care.

  • Develops a comprehensive treatment plan and coordinate the delivery of services to patients enrolled.Coordinates activities related to comprehensive discharge planning for patients on his/her caseload, and mobilizes family and community resources to support the patients¿ successful functioning after discharge from the program. Documents interventions, both direct and indirect, in the patient¿s medical record in compliance with the Division, Department, and Hospital policy.

  • Assists in recruitment and evaluation of performance of therapists including interviewing, quarterly audits of the electronic medical record, and yearly performance evaluations

  • In partnership with the Program Director, may facilitate interdisciplinary treatment team meetings as well as other general staff/therapist meetings

  • Attends Division, Department, and Clinic level staff meetings. Ensures that Division, Department, and Clinic policies and procedures are implemented with regard to professional practice and patient care.Ensures timely completion of statistical reports/forms, time sheets, and other Hospital or Division mandatory in-service trainings and Health Assessments.Subscribes to maintaining a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties.

  • Provides back up social work services as requested and able to other unit/programs during the absence of certain co-workers.

Other duties as assigned

QUALIFICATIONS:

  • Master¿s Degree in Social Work from an accredited program required.

  • 5 years of post-graduate clinical experience working with individuals with chronic mental illness and/or community mental health preferred.

  • 1-2 year¿s experience in clinical and/or administrative supervision preferred.

  • Skills in agency-based policy/procedure development preferred.

  • Skills in effective employee engagement and leadership preferred.

  • Knowledge of mental illness preferred.

  • Ability to engage in person centered treatment preferred.

  • Knowledge/skills in various therapeutic interventions including CBT, DBT, motivational interviewing preferred. Knowledge of risk evaluation and mitigation preferred.

  • At minimum, licensure as a Licensed Clinical Social Worker, as defined by the State of New York Education Department. (LCSW/LCSW-R) required.

Pay Range

Pay Range: $69,326 - $90,126 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
