Description

General Description

Providing administrative oversight and ongoing support to the Program Supervisor (PS) in the Traumatic Brain Injury (TBI) Waiver and Nursing Home Transition and Diversion (NHTD) Waiver Service Coordination programs. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.

Essential Duties and Responsibilities

  • Provides support to supervisor and staff including but not limited to, hiring, training, and ongoing staff development.

  • Completes administrative tasks as assigned including but not limited to Housing, UAS, etc.

  • Promotes and encourages team building within the Care Coordination department and throughout the agency.

  • Participates in and facilitate as needed, staff meetings within the Care Coordination Department.

  • Reviews paperwork completed by staff as assigned by PS to ensure the highest level of detail and quality to meet regulatory requirements.

  • Participates with other members of the supervisory team within the Care Coordination Department and provide coverage for staff and/or the Director as needed.

  • Participates in planning and implementing quality management activities within the Care Coordination Department.

  • Provides direct service coordination to a limited caseload as needed, maintaining accurate and verifiable documentation of all services rendered.

  • Participates in and facilitates (as needed) team meetings and case reviews.

  • Monitors staff-assigned caseloads to ensures accurate capacity.

  • Provides the Program Supervisor or Department Director with ongoing data regarding caseload capacity.

  • Completes other paperwork required by CCFCS, DOH and all applicable funders.

  • Assists with periodic client file reviews ensuring ongoing compliance to all applicable DOH regulations.

  • Serves as a CCFCS representative with networks and/or providers of all applicable programs.

  • Assists in the development, interpretation, and implementation of guidelines and regulations as required by DOH.

  • Attends mandatory training.

  • Represents CCFCS on appropriate boards, committees, and task forces.

  • Ensures compliance with all local, state, and federal regulations, and agency policies.

  • Participates in relevant Agency meetings and/or trainings

  • Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.

  • Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse

  • Other duties as assigned

Qualifications

Education and Experience:

Bachelors degree in human services or related field and one year of care management or case work with persons with disabilities

OR

Associates degree in human services or related field and three years of care management or case work with persons with disabilities

Additional Requirements:

  • A minimum of two years of supervisory experience preferred

  • Bilingual (English/Spanish) is required for some positions, If position requires bilingual skill to be used, additional compensation will be offered

  • Ability to prioritize assignments, plan, and complete work projects with minimal direction

  • An ability to work efficiently and effectively and meet deadlines,

  • An ability to work under pressure

  • Excellent verbal/written skills

  • Demonstrate ethical business practices, in conformance with all state and federal laws and regulations

  • Previous experience working with diverse populations, including low to moderate-income families

  • Ability to travel in all weather conditions throughout the counties served

  • Willingness to foster agency, department and program wide cooperation and teamwork through use of positive/constructive communication techniques

  • Ability to maintain absolute confidentiality regarding all records reviewed including consumer records, employee records and billing records

  • Proficiency and experience with Microsoft applications

  • Ability to analyze and interpret data and to handle problem resolution

  • Continuous use of a reliable, registered and insured vehicle

  • Demonstrate full adherence to the CCFCS Code of Conduct and all policies/procedures related to compliance

  • Ability to demonstrate behaviors and attitudes which support agency mission, philosophy, and policies

  • Possession of a valid NYS Driver’s license and use of a registered and reliable vehicle
