22-24.50 per hour commensurate based on experience | Rochester, NY, USA | Hourly | Full Time
SUMMARY: Provides nutritional and food preparation consultation to staff and people served. Functions and collaborates as a member of the multidisciplinary team. Provides teaching and training as needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
Collaborates with agency contracted menu services. Acts as a champion for the service and facilitates training and education about the program. Assists and supports participating in residential sites with the use of the associated services.
Promote well-being and healthy eating. Conduct audits of program participation, provide menus, and diet consistency orders at participating residential sites.
Conducts mealtime observations routinely and provides reports of observations. Provides coaching and/or immediate training when needed.
Assist with data gathering of nutritional information as needed or requested by agency contracted menu service or healthcare team.
In consultation with the Registered Nurse and multidisciplinary team, monitors specialized diets per provider order as needed.
Participates in New Employee Orientation and/or Residential/Day Habilitation site hands on training on food preparation as requested and as needed. Assists with the lunch program at day habilitation as needed.
Serves as a positive role model for individuals. Interacts with others in a way which will assist the individuals in developing behaviors, skills and attitudes necessary for successful community living. Maintains a neat appearance, hygiene and grooming. Promotes positive behaviors and interactions with all individuals.
Participates in committee meetings as assigned.
Performs other necessary duties relevant to the position as requested.
EDUCATION and/or EXPERIENCE : High School Diploma or equivalent. LPN or associate's degree in health care field/nutrition preferred. Experience with the MY 25 program and working with people with developmental disabilities a plus
Schedule: Mon-Fri 8:00am-4:30PM
