Job Description:

Interview the patient and/or caregiver in preparation for the in person geriatric assessment. Telephone or two-way video conferencing with patients and/or caregivers and the care team in follow-up care plan interventions.

MA is primarily responsible for performing telephone preparation for visit and collaborative geriatric care plan development and implementation. Additionally, the MA will preform in home visits to gather medical information to assist in creation of care plan interventions and complete documentation of visits.

Salary Range:

$18.80-$23.50

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Job Requirements:

Education: Associates Degree in Medical Assisting or successful completion and certification from an accredited Medical Assistant program or successful completion and certification from an accredited Emergency Response Technician program. In lieu of degree or certification a minimum of two years of experience as a Patient Care Technician or Medical Assistant.

Experience: Based on education requirement met.

License/Certification Required: Based on education requirement met

Skills: Effective interpersonal communication and problem solving skills. Ability to work collaboratively and courteously with others
