Health educator

General Description

Provides education to clients living with HIV/AIDS about HIV transmission and how to reduce the risk of HIV transmission. This includes sharing information about medical and psychosocial support services and counseling with clients to improve their health status. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.

Essential Duties and Responsibilities

  • Screens clients for health education needs, creates health education plan and completes intake and assessment for clients enrolled in health education.

  • Conducts assessment and health education plan in the areas of HIV Education, risk reduction, treatment as prevention, medical treatment information, adherence counseling and monitoring.

  • Develops health education curriculum and facilitates education groups to HIV+ individuals intended to increase their independence in their medical care activities.

  • Maintains an individual caseload of HIV+ clients who need specialized attention in the areas of HIV and treatment adherence education, medication adherence, education on health care coverage options, and health literacy.

  • Assists with engaging clients into care and treatment to improve their health outcomes and achieve viral suppression.

  • Addresses and works to remove barriers to enable client access to all necessary components of health care, including but not limited to mental health and substance use services.

  • Maintains high quality and timely documentation in electronic databases, including but not limited to AIRS and Welligent.

  • Completes and submits encounters in electronic database entries to supervisor weekly to ensure accuracy.

  • Meets with each client and maintains telephone availability for clients based on client need

  • Recognizes and embraces diversity through supporting and encouraging the strengths of each individual served.

  • Works collaboratively with the SS team to engage clients to receive appropriate care, treatment, and supportive services with the overall client centered goal of viral suppression and self-management.

  • Conducts and/or participates in multidisciplinary case conferences, coordinates services and referrals with both service and clinical providers that facilitate a client’s engagement and retention in care

  • Participates in relevant Agency meetings and/or trainings.

  • Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.

  • Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse.

  • Other duties as assigned

Qualifications

Education: Associate degree, B.A. or B.S. preferred, with 2 years’ experience working in the field of HIV/AIDS, behavior health, substance abuse, health education, or other chronic illnesses.

Experience: At least 2 years of experience
