HCR Home Care RN (BSN) - Clinical Manager Fulltime In Office in Rochester, New York

Role and Responsibilities

The Clinical Manager supervises a potentially interdisciplinary team of nurses, therapists and social workers, to assure appropriate clinical s ervices are provided by the Agency. This person, or similar alternate, is available at all times during operational hours and participates in activities with respect to professional services. Collaborates between departments to facilitate and assure the delivery of high-quality patient care.

Essential Functions

Supervises the day-to-day clinical operations (including ensuring workflow tasks are completed even during staff absences) and all required patient care is completed including , but not limited to:

  • Review and follow-up on complaint forms and all QI reports .

  • Onsite supervision of clinicians as assigned .

  • Oversee the instruction of the clinical team in providing quality service and maintaining standards, including in-home visits to direct and evaluate patient care .

  • Proactively address operational and clinical practices to prevent patient harm and/or organizational deficiencies or inefficiencies .

  • En sure that the patient’s plan of care is executed as written and reassessed by the appropriate health care professional when there is a significant health status change in the client’s condition, at the physician’s request , and after hospital discharge .

  • Recommend policies, procedures and/or protocols based upon industry regulations and evidence-based clinical practice standards .

  • Assure that staff perform according to Agency standards as well as legal/regulatory standards .

  • Conduct case conferences (informal, supervisory or interdisciplinary) to discuss client issues, plan of care , and discharge planning , including documentation of such conferences via the Case Conference Coordination Note in the medical record .

  • With the assistance of the Clinical Field Staff Supervisor and Patient Scheduling Coordinator, oversee the delegation of assignments to staff according to licensure, training, and demonstrated skills, as well as the clinical needs of the patient .

  • In collaboration with the HR Department and Regional Director, participate in recruitment of qualified personnel and implementation of effective strategies to promote staff retention at acceptable parameters established by the Agency .

  • Ensure clinical staff members maintain up-to-date credentials and adheres to job responsibilities and productivity requirements by conducting timely , thorough performance reviews annually, and providing more frequent feedback on low performance as needed.

  • Assist in determining appropriate staffing levels and maintain adequate staffing levels to meet patient requirement s .

  • Approve weekly schedules and ensure productivity standards are met .

  • Review and approve bi-weekly payroll .

  • Recommend and collaborate on in-service education programs to promote staff development .

  • Take administrative on-call, when required .

  • Maintain nursing knowledge base to be current with accepted community standards and best practice s .

  • Participate in community awareness programs as necessary .

  • Understand and effectively interpret operational budgets and respond to negative variances .

  • Create strategies to meet corporate goals , including team objectives that are attainable. Monitor and evaluates effectiveness using measurable criteria .

  • Provision of monthly staff meetings to meet agency communication plan .

  • Adhere to and participate in Agency’s mandatory HIPAA/Privacy Program and Employee Compliance Program .

  • Read and adhere to all Agency policies and procedures and follow the Employee Handbook Guidelines .

    Education Requirements

  • Bachelor’s Degree in nursing required .

    Qualifications and Requirements

  • Current, valid RN, PT, OT, MSW license issued by NYS Department of Education required .

  • Minimum of two years’ experience in home health care .

  • Medicare home health experience and management experience .

  • Organized, detail-oriented , and possesses effective communication skills both orally and in writing .

  • Understands the delivery of home health care and is able to problem-solve effectively .

  • Capable of prioritizing and handling multiple tasks of critical importance simultaneously .

  • Functions well in an atmosphere of change and interruption .

  • Effective management skills .

  • Must possess a valid NYS driver’s license and automobile liability insurance .

  • Must have dependable transportation and able to drive in all types of weather .

    Work Environment

    The Clinical Manager is primarily in an office setting and may be exposed to outdoor conditions.

    The working conditions are classified as light work:

  • Light work - Exerting up to 20 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Light work involves sitting most of the time.
