Job Description:

Under general direction of the Nurse Manager, the RN Care Manager’s central role integrates and coordinates access and utilization management, proactive patient management, care facilitation and treatment planning functions. The RN Care Manager manages clinical aspects of patient centered care, working with the interdisciplinary medical team in the provision of patient care. May participate in supervision of other clinical staff members.

  1. Coordinates medical care of patients identified during telephone triage with the oversight of APP and Physician clinical determination.

  1. Utilizes professional nursing assessment skills to assess and appropriately triage complex cardiac patients and their medical concerns.

  2. Enhanced critical thinking skills regarding the urgency of patient needs.

  3. Works in tandem with staff to schedule office and referral appointments.

  4. Facilitates referrals to Home Care agencies, and additional ancillary services including acquisition of medications, supplies and equipment to achieve treatment goals specified in the patient plan of care.

  5. Routine and timely documentation of all relevant activity in the electronic medical record. Interacts effectively with physicians, the home care team, patients, and their caregivers.

  6. Assists in developing and implementing population-based strategies to close gaps in medical care.

  7. Identifies resources for patient self-management skills.

  8. Assists in developing and implementing care plans for medically complex patients with some independence as well as in a team based approach with other providers.

  9. Identifies barriers to a successful care management path.

  10. Assists with prescription refills and communication with pharmacies.

  11. Obtaining prescription prior authorization utilizing multiple resources including online insurance management tools.

Manages FMLA and disability paperwork completion for patients in the

Program Development

  1. Assisting with planning short-range and long-range program goals for case management plan.

  2. Keeping abreast of organizational developments and practices that may impact operations by working closely with Lead APP and case manager.

  3. Participating in continuing education courses, professional organizations and seminars, reading current literature and maintaining professional contacts in the community.

  4. Assisting with the development of current evidence based protocols, policies, work flow/flow sheets, guidelines, etc. related to the provision of care within the medical home model.

  5. Participating in committees as assigned.

  6. Conduct home visits for discharge patients that are not eligible for home care service.

  7. Assisting with training and oversight of RN staff prescription refills.

Clinical Practice Responsibilities

  1. Performs direct patient care as needed including managing office visits in the cardiology department, assisting with IV insertions and medication administration.

  2. Assists with direct patient care for both nuclear and stress testing patients.

Salary Range:

$64,480- $89,842 annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Job Requirements:

Fulfilled the educational requirements to be a licensed RN in NYS; BSN preferred. 3-5 years professional nursing experience. Outpatient primary care and/or pediatric (if applicable) experience preferred in Outpatient settings. Progressive clinical experience including: community health, care management, and/or disease management is preferred. Experience working as a member of a multi-disciplinary team is required. Supervisory experience is desirable. Experience in data analysis and chronic illness registries. Current and valid license to practice as an RN in New York State. Current CPR certification required. Valid New York State driver’s license and the ability to travel may be required for positions in an Outpatient setting. PRI certification preferred for positions in Case Management.
