Description

The Department of Medicine, General Medicine Division, Transitional Care Medicine, is seeking a part-time board certified Internal Medicine-Pediatrics or Internal Medicine physician with an interest in evidence-based clinical service and research in the area of care for adults with diagnoses that originate in childhood (i.e. “Transitional Care Medicine”) including autism spectrum disorder, and other intellectual and developmental disabilities (IDD), congenital heart disease, metabolic disorders, spina bifida, sickle cell disease and cystic fibrosis. This physician will be an integral member of the UR team at the Complex Care Center, part of the Division of Transitional Care Medicine, supporting the inpatient and outpatient care of our patients. The Complex Care Center (CCC) is an innovative interdisciplinary primary care setting targeting adults with pediatric onset conditions. Services at the CCC currently include primary care, nutrition, respiratory therapy, behavioral health, pharmacy consult, care management, and dental services. The Complex Care Center offers numerous opportunities for clinical and health system research, regional engagement in health system reform, and teaching opportunities. This role offers the opportunity to provide inpatient and outpatient care and consult support to this patient population while fully integrated with the outpatient interdisciplinary team, outpatient hospital follow up and workforce development for this patient population.

The position may include a combination of outpatient clinical/primary care and inpatient rounding on admitted patients at Highland and Strong Memorial Hospital, clinical care in the outpatient setting and 20% admin time for duties otherwise assigned by the division chief.

Applicants must possess an MD, DO or equivalent and be board certified in Internal Medicine. Board certification in Pediatrics also preferred. Must be immediately eligible for licensure in New York. Experience working with the targeted “transitional” patient population is preferred. Interest in working in an evolving fully integrated program is required.

Eligible for a Faculty Appointment at the University of Rochester School of Medicine and Dentistry

Eligible for Board Certification in Internal Medicine (required) and Pediatrics (preferred)
