University of Rochester Assistant Director, RN Clinical Care Management, Primary Care in Rochester, New York
GENERAL PURPOSE:
Care Management is an essential component of Primary Care¿s population health strategy, designed to improve quality and patient outcomes while decreasing cost.
This position is responsible for executing Primary Care¿s care management strategy ¿ which includes transition care management and complex care management goals. The overarching responsibility for this role is to lead care management activities to create more coordinated, integrated, and whole-person-oriented care.
This position plays a significant role in dovetailing care management across multiple programs, including the Patient-Centered Medical Home, Making Care Primary, and various quality improvement initiatives focused on prevention and chronic condition management.
This role is responsible for day-to-day care management operations and leading a team of 30+ RN Care Managers across two affiliates: the Strong Memorial Hospital and Highland Hospital.
RESPONSIBILITIES:
Develops and Communicates Care Management Strategy
Develops care management strategies, in conjunction with the Medical Director of Population Health and Quality, that align with PCN's strategic imperatives.
Creates plans with SMART goals and key milestones.
Aligns these goals to support the overall PCN strategic priorities.
Routinely reports progress achieved and challenges encountered.
Ensures PCN Care Management program is aligned with AHP¿s Care Management strategic priorities.
Drives performance on Making Care Primary, Patient-Centered Medical Home, and other quality or value-based initiatives.
Works collaboratively with pharmacy, social work, health home care managers, and community partners to achieve the strategic goals of MCP and PCMH.
Assures care managers understand the key goals and metric measures.
Communicates with physicians, SMDs, and Practice Managers to proactively identify needs and necessary clinical and operational improvements with population health initiatives.
Collaborates with the PCN Clinical Operations and PCN Operations Team to support effective practice utilization of CM-led workflows.
Partners with PCMH and MCP staff to ensure tight coordination and success on measure sets.
Aligns care management activities with data analyst initiatives.
Care Manager Performance Tracking
Develops reporting tools that allow Care Managers and PCN leadership to track performance against PCN¿s strategic goals
Ensures reporting and performance of reporting also meets AHP¿s strategic priorities
Collaborates with ISD¿s Ambulatory Epic team to optimize Compass Rose.
Develops and monitors adherence with documentation standards.
Tracks clinical productivity and offers volume projections for staffing, quality improvement and other purposes to support initiatives for which CM is accountable.
Care Manager Leadership, Training and Oversight
Manages and executes training initiatives and has direct supervisory oversight of Primary Care Network (PCN) practices RN, Clinical Care Managers in the following areas:
Develops Care Manager clinical workflows at the practice level, including best practices.
Provides continuous support, leadership, and problem-solving strategies to Network practices.
Performs ongoing competency development and evaluations.
Manages recruitment activities by working collaboratively with Human Resources staff to screen and interview potential candidates
Oversees all performance issues and make recommendations/corrective action as needed in collaboration with Human Resources.
Performs regular site visits to meet with Clinical Care Managers one on one.
May perform other duties as assigned or be assigned
QUALIFICATIONS:
Associate¿s degree in Nursing required
Bachelor of Science in Nursing
Relevant master¿s degree (e.g. Clinician Nurse Leader, Health Systems Leadership) preferred
5 years of professional nursing experience required
Experience working as a member of a multi-disciplinary team including team collaboration with physician engagement
1-year supervisory experience preferred
3 years progressive clinical experience including:community health, care management, and/or disease management preferred
Strong analytical ability and attention to detail required
Strong computer skills and knowledge of Microsoft Outlook, Word, and Excel required
Experience/proficiency using an electronic medical record required
Excellent interpersonal, communication (written and verbal) and organizational skills.Must be able to effectively deal with difficult situations in a poised and highly professional manner. Ability to function effectively with ambiguity and fast paced environment required
Registered Professional Nurse license in New York State required

Pay Range
Pay Range: $84,552 - $118,373 Annually
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
