ADULT PROTECTIVE SERVICE WORKER I

Under supervisory oversight, performs complex Adult Protective service investigations, social work assessments and case management and/or receives and investigates reports of suspected abuse, neglect, self-neglect, exploitation of a vulnerable adult, or perpetrator based abuse under the NH Protective Services to Adult Law, RSA 161-F:42. Develops, recommends, and implements a case plan designed to protect vulnerable adults and prevent future abuse, self-neglect, and/or perpetrator-based abuse, neglect, or exploitation.

ADULT PROTECTIVE SERVICE WORKER II

Performs complex Adult Protective service investigations, social work assessments and case management and/or receives, reviews, and investigates reports of suspected abuse, neglect, self-neglect, exploitation of an vulnerable adult, or perpetrator-based abuse under the NH Protective Services to Adult Law, RSA 161-F:42. Develops, recommends and implements a case plan designed to protect vulnerable adults and prevent future abuse, self-neglect, and/or perpetrator-based abuse, neglect, or exploitation.

ADULT PROTECTIVE SERVICE WORKER III

Performs increasingly complex and complicated Adult Protective service investigations, interventions, social work assessments and case management and/or receives, reviews, and investigates reports of alleged adult abuse, neglect, self-neglect, exploitation of vulnerable adults or perpetrator-based abuse under the NH Protective Services to Adult Law, RSA 161-F:42. Develops, recommends, and implements a case plan designed to protect vulnerable adults and prevent future abuse, self-neglect, and/or perpetrator-based abuse, neglect or exploitation. Provides supervision for assigned subordinates.

RESPONSIBILITIES:

ADULT PROTECTIVE SERVICE WORKER I

Receives, records, and investigates complex reports of alleged adult abuse, self-neglect and/or perpetrator based abuse, neglect, and exploitation utilizing basic knowledge of the principles of investigation and assessment.

Performs basic clinical interviews with the alleged victim, alleged perpetrator and collateral contacts in a variety of locations to gather information regarding the victims situation and the allegations of self-neglect or adult abuse.

Utilizes approved assessment instruments and clinical skills to conduct social work assessments of alleged victim to assess functional status, risk factors, priority level, and crisis resolution interventions.

Identif es potentially problematic and dangerous situations during the investigative process, makes an initial assessment for imminent danger of the alleged victim and seeks immediate supervision for remediation. Activates emergency response if necessary by contacting emergency personnel, notifying family members and/or other service providers.

Maintains documentation of evidence of adult abuse, neglect, self-neglect and exploitation in accordance with program policies.

Develops, recommends and implementsa case plan based on the findings and outcomes of approved assessments. Provides case management for victims of self-neglect, neglect, abuse and/or exploitation. Develops a comprehensive protective investigative report, indicating the outcome of the investigation.

Makes referrals to law enforcement agencies as necessary. Works jointly with law enforcement, courts, and various related administrative, regulatory, and legal entities as it relates to the investigation, provision of services, case plan development, and in accordance with APS policies. Collaborates with the Office of Legal and Regulatory Services, Office of Behavioral Health, Bureau of Developmental Services, LTC Ombudsman Office, and the Board of Nursing.

With supervisory input, develops skills regarding providing testimony in legal hearings and in performing activities necessary to secure needed Guardianship.

Provides partial supervision of interns including providing training and instructions, and assigning and reviewing work.

Participates in program development to assess community needs and resources.

ADULT PROTECTIVE SERVICE WORKER II

Receives, records and investigates complex reports of alleged adult abuse, self-neglect and/or perpetrator based abuse, neglect, and exploitation utilizing knowledge of the principles of investigation and assessment.

Performs clinical and collateral interviews with alleged victim, alleged perpetrator and collateral contacts in a variety of locations to gather complex information and data regarding the allegations and overall alleged victims situation.
