Job Summary and Qualifications

Responsible for the proper implementation of all pharmacy policies and duties inherent to a Licensed Pharmacist, specifically requiring good decision-making skills, self-motivated, resourceful, and independent thinker.

  • Assesses all physician orders to ensure accuracy, drug allergies, drug-drug interactions, appropriate dose for age/weight of patients.

  • Documents all interventions. Communicates well with all health care workers.

  • Counsels' patients on medication use when requested on discharge of a special care nursery patient, a peds patient and all other patients discharged on seven (7) or greater medications.

  • Monitors drugs with a narrow therapeutic range problem prone or high use and makes recommendations to ensure positive patient outcomes.

  • Reviews antibiotic therapy for appropriateness of culture and sensitivity reports as well as cost effective alternative recommendations.

  • Ensures medications are stored and handled properly by completing unit inspections by the end of each month.

  • Assists in teaching programs involving nursing personnel and other health care providers.

  • Provides drug information as requested by patients, nurses, physicians and other health care provides

  • Directs work of technicians including IV admixture, preparation of intensive care nursery meds, TPN, chemotherapy, and unit dose prepacking.

  • Attend codes providing pharmaceutical expertise to the situation.

  • Monitors all medications for patients 65 years or older with decrease creatinine clearance.

  • Review and prepare all special care nursery and pediatric patients per policy.

  • Checks exchange medication carts prior to delivery.

  • Fills/checks employee prescriptions ensuring all documentation completed, counsel's employee.

  • Always reorders when stock is low.

  • Makes rounds at least twice a shift - ensures rounds made at least every hour.

  • Does a narcotic inventory 100% of the time as per protocol.

What qualifications you will need:

  • Education: Graduate of a college of pharmacy with BS or Pharm. D accredited by the American Council on Pharmacy Education.

  • Licensure : Licensed Pharmacist in the Commonwealth of Virginia

  • Certification: BLS

  • Experience: Previous experience as a Pharmacist (hospital pharmacy experience preferred) Organizational skills to handle multiple activities; self-initiative, independent. Computer experiences preferred. Clinical experience preferred.
