Job Summary and Qualifications
To provide navigation services to patients, families, and caregivers facing the impact of a cancer diagnosis throughout the continuum of care. To assure these appropriate services address physical, psychological, social, spiritual, and financial support needs that result from a cancer diagnosis and help ensure the best possible outcome.
Job Functions
Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies.
Serve as an essential link between patients and all other care providers.
Facilitate appointments for consults and support services within established service standards, including case management, dietitians, physician, etc.
Develop patient education programs and tools.
Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management resources.
Works effectively with the multidisciplinary team to meet the needs of the patient/family.
Evaluates and manages the practical problems such as housing, food, financial assistance, employment issues, language and cultural barriers.
Evaluates and assists patient/family with psychosocial issues including diagnosis, treatment decisions, quality of life issues, transitions in care, advanced directives and coping skills.
Provide individual and family counseling, including brief therapy and crisis intervention. Facilitate cancer support groups.
Provide education regarding their disease and any other information that helps to support and empower patients/family.
Collaborates with other community agencies to promote health, provide education and develop better programs in the community.
Documents patient interactions, interventions and services provided.
Demonstrates initiative and provides leadership in the development of new services and programs in response to patient needs.
Attends and participates on the Cancer Committee and attends the Cancer Conference when schedule permits.
Deliver quarterly report card to Cancer committee and Breast Leadership Council and other groups which demonstrate outcomes and performance improvement activities
Demonstrates excellent communication skills working daily with physicians and other healthcare professionals.
Serves as an advocate for patients and families.
Keeps patient information confidential.
Will monitor patient care across the continuum through follow up with patients, families and community services.
Other duties as assigned.
What qualifications you will need:
QUALIFICATIONS
Education: Graduate of an accredited school of Social Services. MSW degree preferred.
Experience: Previous clinical experience in oncology social work preferred. Practicum experience or work experience in medical/acute care, clinical setting required.
