SUMMARY:

The purpose of the Community Health Worker (CHW) is to help patients find a path to better health through education, rapport, resources, and empathy.

Community Health Workers (CHW) are trained public health educators improving health care delivery requiring integrated and coordinated services across the continuum of health. CHWs provide recipients culturally and linguistically appropriate health education to better understand their condition, responsibilities, and health care options. CHW services are related to accessing preventative health services that follow current national guidelines, recommendations, grant requirements, and standards of care. A CHW builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, data collection, social support and advocacy.

Northern Nevada HOPESis a Federally Qualified Health Center and Patient Centered Medical Home.

MINIMUM/PREFERRED REQUIREMENTS:

*Due to real and potential conflicts of interest that exist when HOPES Behavioral Health providers render services to their coworkers, it is HOPES policy (Policy HR0005) that HOPES employees may not access HOPES Behavioral Health Services. For the same reason, current Behavioral Health patients/clients cannot be employed by HOPES. Accordingly, you hereby acknowledge and understand that if you are receiving services from a HOPES Behavioral Health provider at the time you are offered and accept employment with HOPES, you have the option of rescinding your acceptance of employment or finding a new behavioral health provider outside of HOPES before starting employment with HOPES.

MINIMUM QUALIFICATIONS:

  • Bilingual in English and Spanish required.
  • Proven ability to maintain professional conduct, including appropriate boundaries required.
  • Able to multitask, develop organizational structures and set goals required.
  • Able to work autonomously as well as collaboratively with all team members required.
  • Able to exercise empathy and understanding required.
  • Able to communicate and listen effectively required.
  • Prior experience working with target population preferred.
  • Completion of the CHW certification program preferred. Must be completed within 90 days of employment.
