LTCG Supervisor, Care Management in REMOTE, Minnesota

JOB SUMMARY: The Supervisor, Care Management is responsible for providing leadership and technical direction to the Care Management team to ensure that benefit determinations and development of comprehensive care plans are handled promptly, accurately, and efficiently. ESSENTIAL FUNCTIONS Title: Supervisor, Care Management FLSA: Exempt * Provides supervisory oversight of Care Management Staff, including hiring new employees and administering performance management to existing employees. * Mentors direct reports and positions qualifying employees for promotional opportunities. * Creates and maintains process workflows, internal procedures, training curriculums, production metrics and quality review performance measurements at both a team and an individual level. * Reviews existing processes and procedures, as well as system designs, for effectiveness. When necessary, develops a plan and makes the appropriate recommendations for improvement * Ensures inventory levels and individual caseloads are managed in a timely manner to meet service level requirements. * Assists staff with questions and problems related to all aspects of benefit eligibility determinations. * Interacts with clients to resolve concerns with internal procedures, process improvements, inventory levels, quality performance and escalated handling of cases. * Identifies and escalates high-visibility issues to the department manager or director, when necessary. * Attends and facilitates meetings to communicate information necessary to conduct all aspects of business. * Assists in the on-boarding of new clients, including but not limited to Requirement Analysis Document (RAD) discussions, data conversion, plan building, product testing and defining reporting requirements. * Uses time effectively to achieve expected productivity and efficiency standards while prioritizing workloads. * Maintains appropriate organizational confidentiality * Other duties as assigned. List non essential duties here * Demonstrate effective communication skills. * Maintain knowledge of benefit eligibility provisions of multiple long term care policies and plans * Possess extensive knowledge of claims systems, processes and procedures * Demonstrate understanding of company-wide and department-specific policies and protocols Maintain punctual and consistent attendance Minimum Qualifications · BA/BS in Health Related Field or equivalent experience. · 3 years of work experience assisting the older adult population. · 2 to 3 years of progressive supervisory and/or management work experience. · Intermediate level experience with Microsoft Office products. · Required to uphold the principles of compliance as outlined in the Code of Conduct, Employee Handbook and related policies and procedures. Supports and participates in the mandatory Corporate Compliance Program training initiative on an annual or more frequent basis, as required. · Excellent verbal and written communication skills. Certifications Required: current and valid RN or Social Work license Preferred Qualifications · 4+ years of progressive of supervisory and/or management work experience.
