Summary:

  • Under Pharmacist supervision, the Pharmacy Technician Care Coordinator communicates with nurses, physicians, pharmacists, and other members of the medical team to assist patients with medication access and adherence. Roles may include investigation of pharmacy benefits, patient adherence evaluation, copay assistance with manufacturer programs, coordination of recurring medication refills and delivery services for select service lines. Effective communication, critical thinking, and organization skills are necessary for success in this position. The pharmacy technician communicates and coordinates with supervisors, pharmacists, peers, patients/customers, and outside agencies as necessary. The role of individuals in this position facilitates care across multiple sites and transitions in care. The successful candidate will be conscientious and focused on contributing to high quality patient care. (HUP - Radnor Pharmacy)

Responsibilities:

  • Communicate and document appropriately and effectively with supervisors, physicians, nurse practitioners, unit-based pharmacists, social workers, nurse managers, peers, patients/customers and outside agencies.

  • Collaboration with internal team members to gather appropriate paperwork, prescription, billing information, and/or prescription(s.)

  • Coordination of medication access and delivery for outpatient use.

  • Develop and maintain understanding of medication names and drug therapies especially for assigned service lines.

  • Provides appropriate follow-up relating to patients, clinical practices, and family for pharmaceutical needs and/or additional eligibility information to ensure on-going medication access.

  • Proficiently execute all accountabilities of Pharmacy Technician 1.

  • Performs duties in accordance with Penn Medicine and entity values, policies, and procedures.

  • Other duties as assigned to support the unit, department, entity, and health system organization.

Credentials:

  • Certified Pharmacy Technician (CPhT)

Education or Equivalent Experience:

  • H.S. Diploma/GED with 1+ years Pharmacy experience as a certified pharmacy technician in a retail/outpatient pharmacy or payor environment (Required)

  • 2+ years Pharmacy experience as a certified pharmacy technician in a retail/outpatient pharmacy or payor environment (Preferred)
