Responsible for supporting and delivering community health improvement projects. In addition to overseeing identified day to day operations of the individual projects and assisting team members in project planning, project execution and grant writing, position also will create consistency in grant management practices. Maintain and enhance existing community partnerships, diversify partnerships to capitalize on unique collaborative opportunities. Develop overall framework for the hospital's prevention and community facing programs. Consolidate or bundle similar community programs into categories driven by similar desired outcomes or milestones. Responsible for coordinating activities of departmental health improvement projects, monitoring team/departmental tasks to ensure timely completion in accordance with organization and grant regulations.Position provides leadership and expertise functionally and in a matrix capacity with other Community Health leaders. Work collaboratively with outside agencies, service providers, contractors, vendors, senior management, operational managers, and staff to address tasks to meet project timelines. Must be familiar with project scope and objectives as well as role and function of each department team member to effectively coordinate activities of the respective teams. Will manage program budget and prioritize programming to drive budget allocations. This will include assurance that program evaluation includes assessment of community need, ongoing program relevance, and criteria to end a program.

Schedule: Monday-Friday, 8:00 AM-5:00 PM

This is a grand funded role that might need to travel for meetings or events in Pueblo.

Qualifications:

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

  • Bachelor Degree Required

  • Two years' experience in a Community Health worker (CHW) role or in another position with the provision of health improvement services, supportive services, wellness or related duties. Past participation in

  • CHW trainings and grant writing preferred.

  • Possess demonstrated knowledge of use of MS Office programs (Word, Excel, PowerPoint)

  • Working knowledge of programs provided internally and externally by department Working knowledge of health care systems (clinical and non-clinical)

  • Understand and apply project management principle and grant writing. Working knowledge of process improvement tools and techniques

  • Delivery of presentations to various audiences

Sedentary work - prolonged periods of sitting and exert up to 10 lbs. force occasionally
