Summary:

The Intake/Home Health Aide Manager is responsible for the oversight of all referral and admission coordination and home health aide training and personnel to ensure the delivery of quality care, communication/coordination of services, and integration of physician�s orders for the care and services provided to HQHC patients.

Responsibilities:

  • Oversees the intake department with direct reports of the RN clinical liaisons.

  • Oversees the daily home care operations from referrals, SOC, ROC, transfers, and discharges to accurately collect and document required patient information.

  • Serves as a resource to the intake liaisions in the planning and provision of high quality patient care and assists to problem solve complex patient problems through case conferences, education, and home tracer visits.

  • Reviews, hires, and manages all home health aide candidates.

  • Independently runs the HHA educational program and monitors for compliance with NYS.

  • Manages all agency HHAs and the Licensed Agency Cases.

  • Monitors and reports on standards of employee behavior, performance, proactively addresses employees who fail to meet performance standards, and shares with the Director of Patient Care Services.

  • Performs utilization and quality assessment reviews monthly to maintain compliance with Federal and State regulatory requirements that includes record review, transfers, discharges, F2F and OASIS documentation.

  • Responsible for maintaining compliance with HHA supervisions.

  • Runs the Intake Meetings monthly and HHA meetings quarterly. Submits timely minutes to Director.

  • Manages Intake Liaisions, Start of Care Nurses, and Home Health Aides, to include scheduling, PTO requests, and coordination of daily assignments to ensure that patient care needs are met and the plan of care is followed. Coordinates staffing coverage and communicates all changes to the patient, caregiver, physician, and/or representative.

  • Conducts employee performance appraisals within agency guidelines and provides on-going feedback quarterly.

  • Serves on HQHC Quality Assessment Performance Improvement (QAPI) committee and other committees as needed.

  • Exemplifies excellent customer relations with patients, caregivers, physicians, nursing, therapy staff and serves as a positive role model for the department through initiative, involvement, and availability.

  • Administrative on-call responsibilities to ensure 24/7 clinical coverage for agency operations.

  • Conducts annual competences, including handing out of modules yearly to the HHAs and keeps record of progess and reports quarterly to the Director.

  • Maintain and Model REACH Values (Respect, Excellence, Accountability, Compassion, Honor).

  • Demonstrates regular, reliable and predictable attendance.

  • Performs other duties as required.

Other Information:

� Basic Computer EMR skills

� NYS Driver�s License

� Current Car Insurance Policy

  1. Bachelor�s degree in nursing preferred. Graduate degree in public health or health-related field

preferred.

  1. License and current registration to practice as a Registered Nurse in New York State required.

  2. BLS CPR Certification.
