SUMMARY
Creates optimal outcomes for the patient and family by managing complex psychosocial and economic co-morbidities. Utilization Management/Managed Care collaborates with appropriate individuals/department regarding delays in service that may have an impact on quality of patient care and/or length of stay. Assesses patient's clinical course on a daily basis to verify patient's continued need for acute hospital level of care in collaboration with RN Case Manager and primary care nurse. Through practice skills mobilizes resources to reduce patient risks, serves as ambassador between hospital and community, leading the discharge planning process. Ability to be flexible and agile to meet the complex, dynamic healthcare environment while providing world class customer service to patients, families, internal and external customers of the organization. Reports directly to the Lead Case Manager with scope of practice supervision as required if needed by the MSW under the Direction of Assistant V.P of Quality.

ATTRIBUTES
Required: Graduate of an accredited school of social work. Bachelor's degree in Social Work
Preferred: Master's degree in social work
Licensure/Certification Registration Requirements:
Required: Current license as social worker
Preferred: Current NYS licensure as MSW
Required Experience:
Required: 3 years' experience as social worker working in a healthcare environment
Preferred: Experience as a hospital based social worker
RESPONSIBILITIES
- Assessment/Documentation: Assesses patients and integrates information from case managers, physicians and other team members to determine potential and actual risks to recovery, length of stay, appropriate level of care, and discharge planning. Monitors the utilization of observation and inpatient services. Explores strategies to reduce length of stay and resource consumption with optimal patient outcomes.
- Planning: Develops a comprehensive care plan to meet the psycho/social needs of the patient ensuring world class customer service is provided. Discharge Planning Assesses patient/family adaptation to illness/disability and capacity to provide for patient's care needs via biopsychosocial assessment, determine the appropriate continuum of care services as needed. Serves as a resource/liaison, to community social service agencies relative to identified patient/family needs.
- Coordinates/Facilitates: Works with the health care team in discharge planning, collaborating with physicians, nursing, and multiple disciplines, departments, payer, agencies to eliminate barriers to effective discharge planning and addressing pycho/social needs. Collaborates with community social services agencies to enhance discharge planning and continuity of care. Facilitates and coordinates non-medical/social/culturally complex discharge planning, including but not limited to Hospice, Nursing Home Placement and other Community based Social Services.
- Customer Service : Provides Social work services and interventions consistent with regulatory requirements as these related to reporting cases of abuse, neglect, end of life issues and others as needed. Ability to be flexible and agile to meet the complex, dynamic healthcare environment while providing world class customer service to patients, families, internal and external customers of the organization.
- Education: Ensures patients/family/caregivers receive adequate education regarding social work interventions as appropriate to patient's age. Maintains personal growth and development. Participates in staff education on an individual basis and groups as it relates to social work services and programs
- Regulatory: Ensures the timely and seamless transition of patients through medically appropriate levels of care in accordance with licensure, regulatory scope and standards, established hospital polices, and procedures in a patient and family centered manner.
PAY RANGE: $62,000.00 - $75,300.00
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
