Provider Solutions & Development Palliative Care Physician in Portland, Oregon
As a Physician with Providence Connections Palliative Care, you’ll join an established interdisciplinary care team providing inpatient consult service at Providence Portland Medical Center in Portland. In this role, you will work within an interdisciplinary team to provide inpatient consultation to patients with serious, life-limiting illness, who are seeking assistance with advance symptom management, clarifying goals of care, and transition planning. Our Palliative Medicine Physicians works directly with patients and their families and hospital and specialty providers, to create a customized plan of care. Ideal candidates will have completed a Palliative Medicine Fellowship Program, will have at least 3 years of experience in inpatient Palliative care and be either BE/BC in Palliative Medicine. Connections Palliative Care is part of a growing integrated Home and Community Services program which collaborates with Hospice, Home Health, Geriatrics as well as hospital-based practitioners to care for patients and their families.
Part-time at 0.7 FTE
Monday - Friday workweek, daytime hours
Board Certified/Board Eligible Palliative Medicine Physician
Minimum of 3 years of post-graduate clinical experience in palliative care, hospice, geriatrics, and/or oncology care is preferred
Work with colleagues across different specialty areas providing a coordinated, compassionate, multidisciplinary approach
