The Care Coordination Supervisor supervises the Substance Use Assessment Team, Jail Coordination Team, and a Housing Support Specialist. These program areas are new so initially this position will be focused on program development and onboarding. The long term focus of this position is to support these teams in their day-to-day work. The program collaborates with system partners to address the needs of adults with particular emphasis on accessing needed clinical interventions and supports following release from incarceration, individuals seeking Substance Use Treatment, and individuals seeking housing support. As the Care Coordination Supervisor, other duties will include: Program Management Partners with Program leadership to develop and implement the new Jail Coordination Team, Substance Use Assessment Team, and a new Housing and Retention Specialist position. Provides both individual and team based administrative and clinical supervision to the Jail Care Coordination Team, the Substance Use Assessment Team, and a Housing Specialist for the purpose of ensuring quality of client care, use of best practices in service delivery to clients, compliance with county and departmental personnel policies, procedures and work rules, compliance with job performance expectations and accountability, enhancing staff professional development, ensuring an effective referral process, and adherence to professional expectations and maintenance of current credentialing. Research and gather program, local, state, federal and funder-based rules, policies, procedures and requirements to apply to program functions, training of staff, application to team licenses/certifications, and application in compliance with county collective bargaining agreements, Personnel Rules, etc. for any policy or procedure updates or changes needed. Collaborate with program and division leadership to communicate and resolve any conflicts or undefined domain, to clarify to limit program liability and safety concerns. Program Partnership Collaborates with other leadership to provide administrative oversight of the programs and staff including future program development and expansion, program reporting, and customer satisfaction evaluations. Participates in ongoing administrative meetings with multiple county programs, community organizations, and State personnel for the purpose of program review and planning, quality improvement and assurance activities.
