Job Summary:

Performs varying duties related to Social Services and resident care.

Social Workers shall adhere to current federal, state, and local regulations, established policies and procedures.

Assures that the emotional and psychosocial needs of residents are met/maintained on an individual basis and that resident’s rights are upheld.

Essential Job Functions and Responsibilities :

Works as part of the interdisciplinary team to plan, develop, organize, implement, evaluate, and direct the social service programs to support the needs of the residents within the facility policy and procedures.

Participates in discharge planning, development, and implementation of care plans, MDS, and resident assessments.

Maintains HIPAA compliance with resident information and resident rights’.

Ensure privacy when interviewing resident/families as necessary and in a private setting.

Maintain the confidentiality of all resident care information.

Ensures timely reporting and documentation of necessary forms, quality assurance documents, resident care plans, MDS, discharge planning and admission processing.

Encourage resident and family involvement as appropriate in the facility social services programs.

Coordinate social service activities with other departments, staff, community organizations and consultants as appropriate to meet the needs of the resident.

Interviews residents or family members, to obtain social history and other relevant data pertaining to resident.

Acts as a liaison between facility and staff to resident representative, family, resident, and/or to responsible party for issues such as reporting of incidents, room changes, concerns and issues, etc.

Complete MDS and resident assessments as required by Medicare, federal, and state regulations.

Develops preliminary and comprehensive assessments of the social service needs of each resident.

Develop a written plan of care for each resident that identifies the needs of the resident and the goals to be accomplished for each need identified.

Encourages resident participation in development and review of his/her plan of care.

Assist in the scheduling of care plans and assessments to be discussed at each committee meeting. Review and revise care plans and assessments as necessary, but at least quarterly.

Develop care plans that are specific and measurable with use of multidisciplinary staff.

Provide discharge plan of care and preliminary plan of care upon admission.

Reviews and documents complaints and grievances made by the resident and report to the supervisor.

Performs other duties

as assigned.

Qualifications:

    • Preferred Bachelor's Degree in Social Work
  • Preferred Licensed Social Worker in the State of Michigan.

    • Preferred experience in a Skilled Nursing Facility.
