Summary:

Represents Clinical Social Work within Penn Medicine PMC and with external agencies and facilities. Provides supervision to Social Work personnel and Undertakes a leadership role in advocacy, negotiation, mediation and brokering for patients’ rights and entitlements. Performs activities to meet patients’ and families’ needs in a manner that optimizes the discharge planning process and facilitates post-hospital continuity of care within PMPH. Provides counseling, education and support programs that enhance patients’ and families’ ability to cope with physical, emotional and environmental problems. Assists the Director in overseeing the operations related to Social Work.

Responsibilities:

  1. Responsible for optimizing the patient experience, reducing readmissions and meeting financial metrics. represents clinical Social Work on PMC committees and subcommittees.

  2. Develops and sets standards for Social Work practice and advises the Director on Social Work policies and procedures, and other pertinent issues.

  3. Develops and maintains contact with community agencies to gather information on available resources and share information with peers on an ongoing basis.

  4. Supervises PMC Social Work personnel to ensure established Social Work standards and policies, as well as State and Federal regulations, are followed; prepares performance evaluations for PMC Social Work personnel; recommends disciplinary action to Director; participates in counseling sessions with PMC Social Work personnel when personnel fail to comply with Social Work standards, policies, and procedures.

  5. Evaluates staffing and makes adjustments when necessary to ensure that there is sufficient staffing to perform the duties and responsibilities of the Social Work function.

  6. Serves as consultant and resource to hospital staff and community, on an ongoing basis.

  7. Orients new clinical Social Work personnel to clinical Social Work standards, systems, policies, procedures and community resources.

  8. Supervises Social Workers in regards to patients and families with complex emotional, social, and financial consequences of illness and/or disability; Assures advocacy for patient and family empowerment and independence to make autonomous health care decisions.

  9. Provides guidance to Social Work team in regards to interventions in child abuse/neglect, domestic violence, guardianship (temporary/ permanent), foster care, adoption, mental health placement, advance directives, adult/elderly abuse, child protection and sexual assault.

  10. Participates in guiding the discharge planning activities for complex patients, in order to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers.

  11. Provides leadership and guidance to the Palliative Care Social Worker who is responsible for providing on-going emotional support to patients and families, ensuring continuity of care, a medical care plan and discharge plan that are in alignment with the patient and family goals of care.

  12. Ensures appropriate utilization of the palliative care Social Worker and collaborates with the readmission steering committee to address/reduce readmissions related to end of life.

  13. Ensures the Palliative Care Social Worker strives to achieve all health system targets including reduction of readmission rates related to end of life issues.

  14. Promotes individual professional growth and development by meeting requirements for mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor, and resource to less experienced staff.

  15. Assures that the Social work personnel Documents social work interventions and relevant discharge planning information in the medical record according to Department standards.

  16. Collaborates with all departments at Penn Medicine Princeton Health in assuring that patients are transitioned to the optimal level of care in the right location at the right time.

Credentials:

  • Licensed Social Worker (Required)

  • State of NJ Licensed Social Worker LSW required (NJ Licensed Clinical Social Worker LCSW preferred)

Education or Equivalent Experience:

Master of Social Work from a graduate school of Social Work approved by the Council of Social Work

Previous clinical supervisor experience required
