18 months - $3,000

Purpose: This role involves coordinating mental health services for individuals with serious mental health diagnoses. Key responsibilities include assessment, service planning, coordination, referrals, and re-evaluations to ensure community stability in relationships, housing, and employment. The work aligns with the agency's vision, mission, values, and mental health recovery principles, emphasizing a recovery-oriented approach.

Responsibilities:

This role involves coordinating mental health services based on individual needs, often leading a treatment team to advocate for mental health recovery. Key tasks include:

• Building relationships with individuals, their families, and support networks.

• Assessing strengths and needs collaboratively.

• Developing and implementing comprehensive service plans.

• Facilitating interdisciplinary meetings.

• Ensuring access to internal and external resources.

• Providing culturally competent services.

• Maintaining a catalog of community resources.

• Advocating for individuals to ensure service responsiveness.

• Keeping accurate records and documentation.

• Participating in ongoing training and on-call services.

· Bachelor’s Degree in a Human Services field such as Social Work, Psychology or Counseling required. Other fields of study may be considered with coursework of at least 12 credits in Human Services.

· One-year related experience preferred.

· Prior experience working in community settings preferred.

· Safe driving record required.

Licensure, Certifications, and Clearances:

· Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

· Cardiopulmonary Resuscitation (CPR)

· Comprehensive Crisis Management (CCMC)

· Driver's License

· Act 31 Child Abuse Reporting with renewal

· Act 33 with renewal

· Act 34 with renewal

· Act 73 FBI Clearance with renewal

· OAPSA with renewal
