Purpose: This role involves coordinating mental health services for individuals with serious mental health diagnoses. Key responsibilities include assessment, service planning, coordination, referrals, and re-evaluations to ensure community stability in relationships, housing, and employment. The work aligns with the agency's vision, mission, values, and mental health recovery principles, emphasizing a recovery-oriented approach.
Responsibilities:
This role involves coordinating mental health services based on individual needs, often leading a treatment team to advocate for mental health recovery. Key tasks include:
Building relationships with individuals, their families, and support networks.
Assessing strengths and needs collaboratively.
Developing and implementing comprehensive service plans.
Facilitating interdisciplinary meetings.
Ensuring access to internal and external resources.
Providing culturally competent services.
Maintaining a catalog of community resources.
Advocating for individuals to ensure service responsiveness.
Keeping accurate records and documentation.
Participating in ongoing training and on-call services.
· Bachelor’s Degree in a Human Services field such as Social Work, Psychology or Counseling required. Other fields of study may be considered with coursework of at least 12 credits in Human Services.
· One-year related experience preferred.
· Prior experience working in community settings preferred.
· Safe driving record required.
Licensure, Certifications, and Clearances:
· Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
· Cardiopulmonary Resuscitation (CPR)
· Comprehensive Crisis Management (CCMC)
· Driver's License
· Act 31 Child Abuse Reporting with renewal
· Act 33 with renewal
· Act 34 with renewal
· Act 73 FBI Clearance with renewal
· OAPSA with renewal
