REQUIREMENTS:

  • RN-BSN and 1 year of practical experience OR RN and 2 years of practical experience with a background in behavioral health, geriatric and/or developmental delay disciplines.

  • Knowledge of the managed care system, homeless services and community support system experience preferred.

  • Valid PA RN Licensure

  • Act 33/34/73 clearances

  • Pre-employment Drug Test

SCHEDULE: (exempt/salary position)

Monday through Friday with hours as scheduled to best meet the needs of the persons served

ABOUT THE ROLE:

The Nurse Care Manager will provide essential medical and clinical case management services to shelter guests participating in the Mercy of Care (MOC) program. Key responsibilities include:

  • Medication Management: Administering and overseeing medication services.

  • Clinical Assessments: Performing nursing assessments and providing therapeutic interventions.

  • Case Management: Delivering clinical case management and supportive therapy to improve health outcomes.

  • Team Collaboration: Actively participating in the multidisciplinary treatment team to develop individualized treatment plans.

  • Record Keeping: Maintaining accurate clinical records in eClinicalWorks.

This role ensures compliance with applicable laws, regulations, and Pittsburgh Mercy’s policies and standards, fostering compassionate and comprehensive care.
