Job Summary: The incumbent provides routine clinic nursing procedures and personal care services to patients.

Essential Functions: • Responsible for the coordination of patient care and an important participant in policy and procedure implementations as they relate to medical services for nursing staff primarily via the telephone • Evaluates and assesses patients' health care needs based on patients' signs and symptoms utilizing established triage protocols and guidelines • Obtains appropriate input and direction from the provider as needed to determine best clinical course of action and recommendations for specific patient health care • Accurately document symptoms slash complaints, nursing assessment, advice provided and patient slash caller response. Follows policies procedures and protocols to ensure consistency and departmental effectiveness as well as improve healthcare outcomes of patient's callers and their access to appropriate health
Requirements: • Obtains and records patients vital signs and weight • Assist providers with patient examinations • Performs medical procedures and wound care as needed • Administer and document medication administration and patient chart ( oral and intramuscular injection) • Collects urine blood and sputum specimen from patients for testing and performs routine laboratory tests • Reviews providers orders lab requests in close out tasks and providers nurse buckets • Interviews patient to determine medical problem condition and documents in chart for provider • Assist patients with scheduling follow up appointments • Complete and submit prior authorizations for imaging medication Nexplanon ParaGard and other procedures as needed • Follow up on patient request as ordered by provider (example medication refills telephone inquiries from affiliated agencies and other specialty providers as needed) • Maintains health department STD treatment lists and verifications of records • Communicates with affiliated agencies regarding test results and tracks consultation reports • Supports chronic care management team • Assist with stocking and ordering new supplies in disposing of expired or damaged supplies • Assist in administering COVID-19 testing vaccination and patient education • Sterilizes equipment and supplies • Maintains medical equipment and monitors for inspection expiration • Assist in onboarding on new CAN's LPN's and RN's • Participates in community education programs and health fairs • Administers incoordinate flu vaccine clinics • Provides education to patients as needed • Performs other duties as assigned
Qualifications: • Graduated from an accredited School of Nursing as registered nurse or one year of nursing experience in community Health Center or allied health field environment • One year certificate from college or technical school for licensed graduate practical nursing one year of practical experience in a community Health Center or an allied health field environment • Strong customer service focus • Effective verbal communication skills • Teamwork orientation • Organized and able to manage competing priorities • Good judgments • Resourceful and problem solving able to take and follow through with delegated task and accountability
