The Family Practice & Counseling Network (FPCN) provides comprehensive primary care, dental and behavioral health services. Position Summary The Primary Care Site Director is a leadership position that fulfills the role of clinical team leader, mentor, supporter, and clinician resource. In addition, the Primary Care Site Director works to help define a vision of quality improvement, serves as an operations consultant to address Primary Care workflow and direct supervision to Nurse Practitioners, Clinical RNs and Addictions Counselor. Primary Care Site Director position represents the concerns of their fellow Nurse Practitioners at the site level, and brings input into decision-making regarding policy development, standardization of network protocols, CQI priorities, and other issues influencing the experience of clinicians at FPCN. This position has additional administrative time to the clinician's schedule of 5 sessions per week (approx. 20 hours) that will be dedicated to the above-named activities.
Team members are expected to uphold the health center mission by assuring that patients receive health care that is competent, caring, and meets with a high degree of satisfaction. In addition, staff must support FPCN's commitment to the creation of a trauma-informed system of care that continually recognizes and responds to the impact of traumatic stress on all those who have contact with the organization, including children, adults, families, caregivers, and staff.
Essential Duties and Functions Clinical Oversight: 1. Work with clinical staff, Center Director, and Practice Manager to maintain or exceed expected productivity. 2. Ongoing monitoring and adjustment of processes responsible for smooth and effective patient flow. 3. Provide clinical leadership and mentorship for Nursing and Nurse Practitioner practice Share new clinical policies/procedures Chart reviews as needed for audits and QI Patient care issues 4. Promote Quality Improvement processes, , support network improvement initiatives and quality measure goals. 5. Facilitate morning huddles and assist in running clinical staff meetings with focus on primary care team issues and functioning. 6. Encourage and promote teamwork. Nurture individual team members' strengths and abilities. 7. Work with Practice Manager and Director of Primary Care to cover unexpected clinical absences. Assist with coverage issues as needed. 8. Work with Center Directors and Clinical Managers to problem solve clinical workflow issues. 9. Act as a resource and liaison for other members of the site's healthcare team 10. Work in conjunction with Center Director & Practice Manager to promote positive team dynamics through regular meetings and staff supervision. 11. Strive to promote health equity for all patients served across the life course.
Managerial Duties: 12. Supervise: Nurse Practitioners Clinical RNs Addictions Counselor 13. Hold regular supervision meetings with all direct reports to promote productivity, identify strengths and develop individualized plans to address areas of improvement or development, or desired acquisition of skills 14. Routine supervision meetings, at least twice a month, with Director of Primary Care to facilitate smooth communication and promote excellence in patient care services offered on-site. 15. Initiate disciplinary action when indicated. Complete consistent and timely documentation and communication of problems regarding primary care staff. 16. Provide input for MA, front desk performance reviews in collaboration with Practice Manager. 17. Perform annual performance reviews of all direct reports 18. Assist in interviewing and hiring of primary care clinical staff in collaboration with Directors and Managers 19. Onboarding of new nurse practitioners 20. Approve time and CME expenses
Other Functions: 21. Network with other Primary Care Site Directors; meet as needed to ensure standardization of clinical policies and procedures between sites 22. Contribute to the revision and approval of policies, protocols, and network clinical guidelines. 23. Perform other duties as assigned.
Qualifications Master's degree. Graduate from an accredited nurse practitioner program. Current RN and APN licenses from the Commonwealth of Pennsylvania. Current nationally recognized advanced practice certification. Minimum of experience 3-5 years as a Nurse Practitioner required Minimum of 1 year of supervisory experience of Nurse Practitioners preferred. Demonstration of clinical competence. Proficiency in computer skills and electronic health record. Physical and emotional ability to perform the duties as described above. Current CPR Certification Must be COVID vaccinated

The use of arm and/or leg control requires force greater than sedentary, but worker still sits majority of time. Covid-19 Vaccine Mandate: FPCN, a program in the City of Philadelphia, are required to follow the COVID vaccine mandate for healthcare workers per Philadelphia Department of Public Health. FPCN requires Healthcare Workers to be fully vaccinated. Fully vaccinated means it has been two weeks since the final dose of vaccine. Thus, we require vaccination prior to start for all new hires. This currently does not include boosters.
