Resources for Human Development (RHD) is seeking a full time LPN for its NOVA 2 program located in Northeast Philadelphia. This position comes with a generous benefits package and an hourly rate of $29.83 plus a $3,000 sign on bonus: to be paid upon successful completion of 90 days, and paid upon 180 days' successful completion!
NOVA II (a Program of RHD) is a long-term residential program serving adults who have significant medical concerns in addition to a serious mental health diagnosis. RHD employees working in programs within the City of Philadelphia are required to provide proof of Covid vaccination. If not fully vaccinated, they must receive at least one shot in a two -dose series or a single dose in a one-dose series before beginning in-person shifts. Final doses must be received within 8 weeks of hire.
Responsibilities/Duties * Maintains respectful and trauma-informed interactions with all residents, staff, family members and visitors at all times * Provides shift oversight, assuring health and safety of each resident * Maintains nursing documentation and medication records * Assists residents in learning about their medications; assures that medications are taken as physician ordered and recorded accurately in MAR * Provides nursing care including, but not limited to, wound care, trach care, diabetic counseling, etc. * Assesses residents' health and emotional needs * Assists residents with activities of daily living as required
Required Experience * Minimum of two (2) years medical/surgical experience * One year behavioral health experience preferred
Required Education * High school diploma/GED * Must hold a valid LPN license
Required Qualifications * Must be CPR certified * A valid driver's license is required * Ability to stay awake throughout entire shift * Ability to circulate through program space throughout entire shift, including walking up and down stairs between first, second and third levels multiple times per shift * Ability to push adult residents in wheelchairs for extended periods on various terrains * Ability to understand and function within a trauma-informed environment
Program Summary NOVA II provides needed services for those individuals that experience both physical and behavioral health challenges. Individuals reside in a scattered apartments setting, and staff provide mobile supports based on the individual's needs which may include case management, crisis intervention, counseling, medication and nurse monitoring, and skill building, to assist the individual in maintaining his/her independent living. NOVA II serves a five-county area a d specializes in serving people with a history of long-term hospitalizations.
