Description:

The Lead Special Procedure Technologist (SPT) functions independently as a member of the Heart & Vascular Procedure Center patient care team that includes registered nurses, special procedural technologists, residents, cardiology fellows and faculty physicians. In addition to performing the most complex interventional procedures, this position acts as the principal SPT overseeing the technical operations required for the performance of routine and advanced Cardiovascular (CV), electrophysiology (EP), and interventional radiology (IR) procedures. This individual is responsible for coordinating work assignments including on call schedule; reviews and updates procedure manuals and protocols; assists in staff education, maintaining competencies and training, primary preceptor for new technologist; performs proficiency testing; ensures daily quality controls and testing for all equipment, equipment/software superuser, troubleshoots equipment and facilitates repairs; provides input to Manager regarding hiring and annual performance appraisals for SPT objective staff; collaborate with THMA value analysis RN and vendors to introduce new products in alignment with Trinity Health; oversee all patient charges and coordinate with the Revenue Integrity CDM Optimizing Liaison; and assists quality improvement projects in the department. The Lead SPT develops and implements systems to assure the smooth and efficient flow of patients for procedures in the CV, EP, and IR Laboratories. Duties for this Lead SPT position include all of the objective SPT duties but are not limited to: hemodynamic monitoring, interpretation and operation of the Mac Lab/Cardio ComboLab/Carto mapping systems, circulating and scrubbing roles during Invasive Cardiovascular/Radiology, Cardiac Device Implantable procedures, basic electrophysiology and other procedures scheduled in our Heart & Vascular Procedure Center. This position also includes patient teaching, assisting with pre and post patient assessment, patient care within scope of practice, inventory/supply management and schedule coordination. This individual must demonstrate clinical and technical expertise in working with cardiovascular patients during invasive diagnostic and interventional angiographic procedures.

This position requires the individual to be flexible in their work schedule. On-call is required. This individual is considered an expert level SPT, has ACLS, BLS and RT and/or RCIS certification. The ability to direct others and to organize a varied workload is required. This incumbent demonstrates competency in assisting with Interventional procedures from adult thru geriatric patient population. This position reports directly to the Invasive Cardiology Manager. This individual’s annual performance is jointly evaluated with input from the Medical Director of the Cardiovascular/Interventional Radiology Labs.

A. Leadership and Staff Education

  1. In conjunction with the Invasive Manager, Medical Directors, and Radiology Director, supports development and achievement of organizational goals and objectives.

  2. Works to identify and solve operational problems, reduce costs, improve efficiency, and continually improve overall operation of the Heart & Vascular Care Procedure Center.

  3. Assists with On Call for SPT staff including weekdays, weekends, and holiday rotation in collaboration with the department Manager. Submits weekly/monthly on call schedule to WebXchange.

  4. Collaborates with Materials Management to maintain and control the supply and equipment inventory par levels and expense for the department.

  5. Reviews, edits, corrects, and validates Daily Activity Journal (DAJ) to ensure accurate patient procedure and supply charge entry for EP, CV, and IR patient procedure hospital billing daily.

  6. Reviews, edits, corrects, and validates PACS to ensure accurate patient procedure image labeling for IR patient procedures daily.

  7. Assists with planning to modify policies, procedures, staffing and expenses as required by new administrative strategies, CMS and TJC regulations and conditions of coverage or as reimbursement changes dictate.

  8. Works to identify cost effective alternatives for staffing, equipment and supplies while maintaining high level quality of care.

  9. Serves as a clinical and technical expert resource to all staff, fellows, and physicians as appropriate.

  10. Serves as a role model to all personnel by demonstrating a high level of expertise with EP, CV, and IR systems, including trouble-shooting skills, effective interactions with patients & staff, leadership ability and professional behavior.

  11. Precepts newly hired SPT. In conjunction with the Invasive Manager ensures new SPT receives adequate and appropriate training, education, and clinical support during orientation.

  12. Precept and train all staff, including fellows to the scrub role. Assists with cross training nursing staff to the monitoring role.

  13. Participates in the process to ensure established SPT staff perform annual competencies, complete required testing, and have access to appropriate continuing education opportunities.

  14. Collaboration with Invasive Manager, Medical Directors, Radiology Director, and Nurse Educator in planning, guiding, and evaluating technical staff’s clinical skills and competency.

  15. Maintains constructive, positive working relationships with other IP units and support staff.

  16. Promotes and models appropriate open and effective communication among staff, patients, and providers.

  17. Communicates and interacts with all staff, internal and external customers in a professional manner.

  18. Shares responsibility for ensuring a safe and therapeutic environment for patients, families, visitors, and staff.

  19. Creates and promotes a healthy positive work environment.

  20. Provides feedback on staff members as requested by Supervisor and Manager for annual performance evaluation process.

B. Direct Patient Care and Clinical Skills

  1. Implements safe, competent, and efficient procedure care to EP, CV, and IR patients within policies, procedures, and standards of care across a variety of populations and complexity of situations within the scope of practice.

  2. Demonstrates expertise in SPT technical skills, image acquisition, transferring and archiving of images, trouble shooting skills and expert knowledge of catheters and equipment used during EP, CV, and IR procedures.

  3. Participates in Radiation Safety committee with Invasive Manager, Medical Physicists, and Risk Management.

  4. Responsible for initiating, collecting, and monitoring all staff, fellows, and physicians’ monthly radiation exposure information and staff dosimeters in conjunction with the Radiation Safety Officer.

  5. Oversees and actively participates in annual EP, CV, and IR skills, ECG competency and unit competencies for HVC Procedure Center staff.

  6. Demonstrates an expert knowledge of radiological equipment & safety, shielding techniques, expert imaging skills and X-Ray equipment trouble-shooting skills, analyzing and recording equipment and other equipment utilized in the Heart & Vascular Care Procedure Center.

  7. Duties for this position include but are not limited to circulating, scrubbing, and hemodynamic monitoring of patients during EP and CV procedures and other procedures scheduled in HVC. Intra and post procedure patient assessment and patient care within SPT scope of practice, patient teaching, inventory management and schedule coordination.

  8. Systematically and continuously collects, assesses and documents data in collaboration with the multidisciplinary team to provide procedural care for the patient. Documentation of chronological events of the procedure and medical care during the procedure will require basic computer skills to complete electronic medical record.

  9. Performs at an expert clinical level and assists the physician in the performance of adult EP, CV, and IR procedures, including both diagnostic and interventional.

  10. Demonstrates an in-depth knowledge of the effects of moderate sedation process. Assists with patient monitoring of level of moderate sedation in conjunction with procedure performed and individual patient’s clinical state. Initiates additional patient comfort measures/alternatives for sedation, as appropriate.

  11. Assures that informed patient consent has been obtained and that all questions and concerns have been answered to the patient’s satisfaction. Ensures Universal Protocol standards are followed and met for all invasive procedures.

  12. Implements safe, competent, and efficient care to patients within Nazareth Hospital and departmental policies, procedures, and standards across a variety of populations and complexity of situations.

  13. Able to identify adverse events or unanticipated outcomes using critical thinking skills and notifies the medical staff including nurses and physicians appropriately.

  14. Demonstrates knowledge of cardiovascular/electrophysiology medications, vasopressors and anti-arrhythmic agents including dosing, side effects and drug interactions per ACLS guidelines. Able to identify adverse events or unanticipated outcomes using critical thinking skills and notifies the medical staff including nurses, fellows, and physicians appropriately.

  15. Provides direct patient care and serves as a role model and expert resource for both internal and external customers regarding standards of care for the Heart & Vascular Center Procedure patients which include but is not limited to adult diagnostic and interventional cardiac/electrophysiology procedures, adult peripheral vascular and interventional radiology diagnostic and interventional procedures.

C. Quality Care and Performance Improvement Initiatives

  1. Supports medical and nursing management of patient-family centered care daily.

  2. Supports patient advocacy, patient rights and responsibilities and service standards.

  3. Maintains documentation and records, staff competencies, monitors and performs all relevant technical quality indicators as required by TJC standards and regulation for Point of Care Clinical Labs, i.e., Hemochron.

  4. Promotes a unit-based, multi-disciplinary quality improvement program that incorporates patient care standards and meets CMS and TJC regulations.

  5. Collaborates with and contacts outside consultants as needed to ensure equipment functionality, maintenance and other services are performed on schedule.

  6. Performs specialty equipment troubleshooting, repair and maintenance as indicated or instructed by vendor or clinical engineering.

  7. Analyzes and assists in resolving problems resulting in improvement of patient care, practice, and system operations.

D. Professional Development

  1. Develops and plans individual own professional growth and development in relation to expectations for practice in our specialty area.

  2. Attend continuing educational programs, in-services and conferences to maintain and advance expert skills and knowledge level.

  3. Maintain current BLS and ACLS certification.

  4. Participate in ongoing, unit-based quality improvement programs.

  5. Continue to enhance all clinical skills and expertise.

  6. Provide quarterly unit-based in-services, educational programs and training opportunities for staff.

  7. Active participate and member of HVC Procedure Center Unit Based Council.

E. Other

  2. Maintains licensure, certifications and ongoing CEUs for professional development on a timely basis.
