Looking for a Full-Time HIV Prevention Navigator!!

Summary:

  • The HIV Prevention Navigator reports to the Ryan White leaders. S/he is responsible for assuring quality HIV Prevention services are provided at Penn Medicine specifically promoting the use of PrEP (Pre Exposure Prophylaxis) and PEP (Post Exposure Prophylaxis) for prevention of HIV infection. The HIV Prevention Navigator will work directly with patients currently prescribed PrEP as well as promote to use of PEP and PrEP with new patients. The Navigator will field incoming calls to the PEP Center of Excellence (COE). The Navigator, along with the HIV Prevention team at Penn (HIV Lead Physicians from PPMC and HUP) will establish a campus-wide campaign to promote PrEP at Penn and work with designated Medicine practices who want to expand their prescribing of PrEP. The Navigator will also work closely with the HIV Care Navigators and HIV Lead Physicians to work to promote PEP and PrEP in the Emergency Department and Medicine practices. S/he is responsible for implementing and assuring that proper documentation and activities are in compliance with appropriate agency, city and funding regulations as stipulated by the Philadelphia Department of Public Health. This is a grant funded position and continuation of the position is 100% contingent upon continued grant funding.

Responsibilities:

  • Support the HIV prevention program to promote the use of PrEP and PEP at Penn Medicine, primarily PPMC and HUP with designated medicine practices.

  • Assess, triage, and provide risk counseling for patients in the City of Philadelphia who call the PEP COE for post-exposure prophylaxis. Work collaboratively with the Infectious Diseases care team (navigators, social workers, physicians) to ensure eligible patients receive PEP within 72 hours of non-occupational exposure.

  • Work with Emergency Department leadership to create a model to ensure appropriate patients are offered HIV prevention services and referrals to PrEP and PEP providers.

  • Provide educational opportunities for ED staff to learn about HIV prevention and the importance of routine offering of PrEP and PEP services.

  • Provide patients with a comprehensive explanation of HIV/AIDS, STI’s and implications.

  • Conduct an accurate risk assessment with clients and thoroughly explain the implications of risk behaviors in regards to HIV infection. Assess patients for PEP.

  • Participate in community-based programming, including (but not limited to) health fairs, AIDS Education Month, and other organizational/departmental priorities.

  • Provide emotional support, counseling, crisis intervention, and advocacy for clients throughout the process of testing for HIV infection as needed and make referrals to PrEP and PEP providers for those who are HIV negative and to HIV providers if positive.

  • Provide comprehensive information on the risks and benefits of HIV testing and provide a through explanation of the testing process.

  • Assist in the tracking and distribution of condoms and safer-sex materials

  • Provide clients with all aspects of post-test counseling, including but not limited to: actual test results, direct referrals, counseling and emotional support.

  • Answer phone calls and respond to emails from the general public and Penn Medicine community by providing information around HIV prevention, treatment, PrEP, nPEP, STI prevention, STI treatment resources, and other relevant sexual health topics.

  • Facilitate and ensure that patients on PrEP and PEP receive the standard of care for medical care associated with taking PrEP.

  • Submit monthly statistical/narrative reports as required to the Program Supervisor for review.

  • Regularly attend and actively participate in staff meetings and staff training sessions.

  • Advocate and interpret for clients as needed.

  • Refer to Penn Community Practice, MacGregor ID Practice, or medical site of their choice for all clients testing HIV positive and those at high risk for HIV infection, for proper follow-up of needed services.

  • Comply with HIPAA regulations and universal precautions for healthcare workers.

  • Reports all incidents to the Ryan White Coordinator.

  • Remain updated on all aspects of HIV/AIDS including approved medical treatments, new prevention methods, etc.

  • Take after-hours call coverage throughout the year for the PEP COE

  • Complete mandated reporter training: Recognizing and Reporting Child Abuse: Mandated and Permissive Reporting

Credentials:

Certification in HIV prevention counseling(Preferred)

Education or Equivalent Experience:

  • Bachelor's Degree Social Work, Public Health or related field and 2+ years Social Service Field (Required)

  • Master's Degree (Preferred)
