Education/Training/Experience: (indicate if required or preferred) Healthcare related masterrsquo;s degree preferred. Case management/transition management experience required. Minimum of five years clinical experience preferred. Minimum of five years progressive hospital management experience with fiscal, operational and supervisory experience highly preferred. Experience with performance improvement and change management preferred (CPHQ, LEAN, Six-Sigma or other performance improvement methodology).

Licenses/Certifications: (indicate if required or preferred) Current registered nurse licensure in the State of Pennsylvania required. Accredited Care Management (ACM) and/or case management certification (CMC/CCM) preferred.

Provides operational oversight of the Care Management Department to assure the delivery of seamless care management across the continuum of care for our patients through the coordination of the right care, at the right time, in the right place, by the right provider. This leadership role requires a highly collaborative, interdisciplinary approach to patient centered care coordination that is safe, reliable, and timely.

Accountable for Care Management program including Case Management, Utilization Management, Social Work and Denials Management mdash; its vision goals, functions, personnel, and outcomes. The Director promotes operational effectiveness while ensuring the provision of appropriate reimbursable care; and oversees the implementation and standardization of best practices throughout the CM department.
