Position Purpose: As a Registered Dietitian/Nutritionist at West Philadelphia Care Ministries, you will play a vital role in promoting the health and well-being of our elderly community members through comprehensive dietary assessments and personalized care plans. Your expertise will ensure that each participant receives the nutritional support they need to thrive independently.
What You Will Do:
Conduct initial and annual comprehensive nutritional assessments for elderly participants.
Collaborate with interdisciplinary teams to develop individualized care plans.
Monitor participant progress and make recommendations for dietary adjustments.
Provide education and training to healthcare professionals as needed.
Oversee dietary aides and ensure adherence to standards of care.
Act as a liaison with meal service providers to ensure dietary needs are met.
Minimum Qualifications:
Bachelor's degree in food and nutrition, dietetics, or related field.
Registered Dietitian/Nutritionist licensure.
Minimum one year of documented experience working with frail or elderly populations.
Strong knowledge of clinical nutrition and therapeutic diets.
Excellent interpersonal skills and ability to collaborate effectively.
