Position Profile
The Clinical Resource Social Worker (CRSW) helps to strengthen individual and team capabilities, efficiencies, and service excellence through education, training, mentoring, and support of social workers in a designated region, clinical office, or department.
This role will serve as CRSW 3 days a week and cover social work needs 2 days a week.
Responsibilities
Provides expert clinical consultation, education, and support to social workers and leadership regarding psychosocial issues of patients and families, community resources, and updates on health care policy affecting end-of-life care.
Participates in the development of processes and protocols related to social work practice to help ensure standards of care and regulatory requirements are met.
Collaborates with the Education Department and leadership to design and develop education initiatives to address agency social worker learning needs.
Utilizes engaging presentation skills to deliver content and adjust presentation style to meet needs of learners in various environments and circumstances.
Facilitates delivery of clear, consistent messaging about HOV’s initiatives, changes, and social work practice expectations across the agency.
Collaborates in the review and hiring of diverse, high-quality social work applicants.
Promotes high engagement and retention by assessing initial orientation needs of all new hire clinical social workers and collaborates with employee and manager to customize the orientation plan to meet individual needs.
Provides ongoing evaluation and monitoring of new employee’s progress, supports growth through feedback and joint visits to observe and assess clinical skills and ensure that essential elements of the role are achieved.
Further improve the quality of Clinical Resource Social Worker leadership by maintaining professional knowledge and enhancing professional skills.
Keeps abreast of hospice regulations and expected service delivery in support of HOV’s strategic plan.
Maintains professional knowledge and enhances professional skills.
Adheres to high standards of personal and professional conducts.
Minimum Qualifications
Master's degree in Social Work
3+years hospice experience
Preferred Qualifications
Minimum one-year adult teaching experience
LCSW or LMSW licensure
APHSW-C Certification
