THE POSITION

DESCRIPTION OF WORK

You will function as a Social Worker in the Continued Care Service of Clarks Summit State Hospital with complete responsibility for providing and participating in social service casework and the community support planning processes that support and promote recovery and transition from the hospital. This position requires a working knowledge of all Social Service and hospital policies and procedures, CMS regulations, and current mental health legislation and resources available to assist patients with day-to-day coping skills and ultimate discharge planning.

Work Schedule and Additional Information:

  • Full-time employment

  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 30-minute lunch. May change based on operational needs.

  • Overtime and travel as needed

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • A bachelor's or master’s degree in social work or social welfare.

  • Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.
