DESCRIPTION OF WORK

As a Registered Nurse Assessment Coordinator, you will perform comprehensive resident assessments, care coordination and planning, and resident advocacy and teaching. You will also advocate for the facilitation of open communications among care team members, the residents, and their families. In this position, you will be r esponsible for the initiation of the baseline care plan and development of the nursing comprehensive care plan, ensuring that it is person centered and in accordance with applicable regulations and guidelines. It is vital for you to m aintain positive, constructive working relationships with co-workers and be aware of the needs and sensitivities of others.

Work Schedule and Additional Information:

  • Full-time employment, 40-hour work week

  • Work hours are 1st shift (8:30 AM to 5:00 PM) , with 30-minute lunch.

  • Every other weekend off.

  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • One year of clinical nursing experience in a long-term care facility including the performance of MDS-based nursing assessments and the development of individualized nursing care plans; or

  • Possession of an active certification as a Resident Assessment Coordinator – Certified (RAC-CT) as issued by the American Association of Nurse Assessment Coordination (AANAC).

    Special Requirements:

  • All positions require active authorization to practice as a Registered Nurse in Pennsylvania.

  • All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.

    Other Requirements:

  • PA residency requirement (https://www.employment.pa.gov/Additional%20Info/Pages/default.aspx) is currently waived for this title.

  • You must be able to perform essential job functions.

    Legal Requirements:

  • A conditional offer of employment will require a medical examination.

  • This position falls under the provisions of the Older Adult Protective Services Act.

  • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
