Rural Naviagator
Job Summary
The Rural Navigator\'sprimary role isto enhance the likelihood of success, identify and reduce barriers for clients and connect them with needed services and resources.The Rural Navigatorwill work with individuals who have been determinedas in need of additional support and engagementand will be responsible for conducting outreach, engagement, case management, and ongoing wrap around supportive services.The Rural Navigatorwill also map out the most appropriate next steps for the individual who may need assistance in accessing various servicesto achieve ongoing success. TheRural Navigatorispart of a multi-disciplinary team made up ofinternal and external stakeholders.The Rural Navigatorwilldocument,and track required patient interactions according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community BridgesPoliciesand Procedures, and specific for the program.
Skills/Requirements:
- Highschool diploma or GED is required. Associates Degree (or higher) in a field related to Behavioral Health is preferred but not required of this position.
- Must be in recovery from Alcohol or Drugs and/or GMH for equal to or greater than 1 consecutive year.
- 1-3years of full-time Health Care related work experience where his or her priorexperience would provide adequate exposure to both behavioral and medicalcrisis situations ispreferredbut not required.
- 1-3 years of substance abuse/mental health recovery-oriented service is preferred but not required.
- Minimum of ninety (90) daysexperiencein aNavigatorIor Peer Support Specialistrole required.
- Must be at least 21 years of age.
- Must obtain a fingerprint clearance card
- Must possess a current valid drivers license. Must be able to provide a 39-month MVR.
