Job Duties
Greets patients arriving for appointments and provides information and resources as needed.Assist patients with scheduling appointments and navigating benefits and will support patient retention in care through follow-up calls and reminders.Responds to patient calls in a timely manner.Provides age and culturally appropriate information and resources during the diagnostic evaluation.Identifies support groups appropriate for patients and families and coordinates scheduling.Removes obstacles to treatment by appointment coordination, referrals coordination, arranging transportation, and other needed services.Tracks and documents metrics and outcomesWorks independently within established procedures associated with the specific job function.Must be able to coach, demonstrate and train.Must maintain confidentiality of private health information.Ensure patients are scheduled for visits within the program and provides information regarding directions and what to expect.Maintains a secure spreadsheet of program patients to track their next visit and remaining medication refills; calls patients to ensure they continue medication and continue to receive appropriately timed visits according to their individual medical needs.Tracks patient attendance for appointments, initiative outreach as well as adhering to missed appointment procedures. As directed by the physician, may provide health education or screenings for psychosocial needs.Provides referrals to community resources as needed and follows up to ensure referral completion.Seeks out additional resources and referrals as needed by program patients; maintains program database of community partners and referrals.For patients seeking surgeries, supports patients throughout the pre-surgical process, such as ensuring timely completion of surgical clearances and medical letters.Conducts outreach on behalf of the program, including attending community events asneeded.Engages patients with patience, empathy, and genuine care, ensuring a positive experience for all.Resolve concerns with professionalism and kindness, maintaining a patient focused approach.Leads with compassion and models’ kindness to promote a collaborative and respectful environment.Other duties as assigned.
Position QualificationsExcellent communication and interpersonal skillsExperience working in a medical or social services environment preferred.Strong attention to detailProficient with basic computer programs, particularly MS ExcelAnalytical critical thinking skillsAbility to manage time, prioritize, and meet deadlines.Ability to work independently and as a team member.Good attitude, professionalism, kindness and promotes a collaborative team approach.Proficiency in Spanish preferredExperience with service coordination preferred.
Requirements of the positionBachelor’s Degree required. At least 3-5 years of related experienceProficiency in Spanish preferred
