A Brief Overview Children's Hospital of Philadelphia is seeking a Licensed Practical Nurse for their Paoli Primary Care office. This position will be Monday-Friday and will require a rotating Saturday shift. The shift will be dependent upon the operational need.

The Licensed Practical Nurse performs routine nursing care of the ill, injured or infirm under the direction of a licensed professional (RN) nurse, nurse practitioner and/or a physician. The LPN participates in the planning, implementation and evaluation of nursing care in accordance with in the scope of the LPN as defined by the State Board of Nursing and health system standards. Also supports operations through administrative duties as required. The nurse must demonstrate competence in caring for patients with moderate to complex health issues and assist the Site Manager with administrative issues which affect the clinical practices within the office as well as comply with the administrative policies and clinical protocols as outlined in policy and procedure manual. Administratively responsible to the Site Manager and Ambulatory Nursing Director for nursing practice issues. Must be willing to rotate if needed to other Care Network sites within the designated cluster.

What you will do Provides and documents direct nursing care of assigned patients under the supervision of a registered nurse or physician. Nursing care is guided by physician orders and the plan of care. Patient response to care is reported to a registered nurse or physician for evaluation, intervention and modification of the plan of care. Assists other health care personnel in the delivery of patient care. Participates in maintaining the environment of care including equipment and other material resources. Participates in own professional development by maintaining required competencies and attending educational offerings. Supports the development of other staff and formal learners. Participate sin the identification of clinical or operational performance improvement opportunities and assists in performance improvement activities. The LPN may administer medications that are with their scope of practice and organizational standards only if the following conditions are met: a) the LPN has received and satisfactorily completed a Board approved educational program which requires study and supervised clinical practice intended to provide training necessary for administering immunizing agents. b) A written order has been issued by a licensed physician pertaining to an individual patient or group of patients, c) the LPN has knowledge of the health system policies and has met competency training standards. Supports clinical activities related to nursing and clinic flow and make recommendations to manager for improvement in quality and effectiveness of nursing care given. Demonstrates commitment to patient safet by speaking up about safety issues and changing nursing practices to enhance safety. Shares errors through system reporting mechanisms, and demonstrates shared accountability for safe practices. Performs other related duties incidental to the work described herein. Education Qualifications High School Diploma / GED Required Technical Diploma Nursing Required Experience Qualifications At least two (2) years medical field Preferred Pediatric experience Preferred Skills and Abilities Computer Competency (Required proficiency) Basic use of a computer (mouse, keyboard, printer, USB ports). (Required proficiency) Basic use of Microsoft Office (Word processing, spreadsheets, presentations and Skype). (Required proficiency) Web browsing, intranet search, document access. (Required proficiency) Use of shared network file space. (Required proficiency) Basic use of patient facing technology. (e.g. IV pumps, point of care devices, CR monitors). (Required proficiency) Navigation and use of clinical communication systems. (Required proficiency) Information Literacy (Required proficiency) Determines the nature and extent of clinical information needed and uses the appropriate technology to access it. (Required proficiency) Accesses needed information effectively and efficiently. (Required proficiency) Evaluates information and its sources critically and appropriately incorporates it into clinical practice. (Required proficiency) Evaluates outcomes of the use of information. (Required proficiency) Information Management Literacy (Required proficiency) Navigation of the electronic health record system. (Required proficiency) Ability to locate and review specific patient data in variousclinical information systems. (Required proficiency) Effectively uses clinical decision support tools. (Required proficiency) Understands policies and procedures, confidentiality, security, and privacy regulations regarding the use of clinical information systems. (Required proficiency) Ability to collect clinical data using quality improvement tools. (Required proficiency) Licenses and Certifications Licensed Practical Nurse (LPN) (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required or Licensed Practical Nurse (LPN) (New Jersey) - New Jersey State Licensing Board - upon hire - Required and Cardiopulmonary Resuscitation (CPR) - issued through on organization that requires a hands-on instructor lead psychomotor skills verification (ex. American Heart Association or Red Cross). - upon hire - Required
