Actalent RN Case Manager in Orlando, Florida

Job Title: RN Case ManagerJob Description

The RN Case Manager will perform telephonic case management for Medicare Advantage members in the state of Florida. This role involves performing care coordination activities, developing care plans and assessments, conducting new referrals, calling members, and building new cases in Athena (EMR).

Responsibilities

  • Perform telephonic case management for Medicare Advantage members.

  • Conduct care coordination activities including developing care plans and assessments.

  • Manage new referrals and build new cases in Athena (EMR).

  • Communicate with members to provide updates and gather necessary information.

  • Handle a caseload of at least 73 members.

Essential Skills

  • Active RN license in the state of Florida.

  • Minimum of 2 years of case management experience from hospital/acute care, home health, or hospice.

  • Experience with Athena EMR software/platform.

  • Strong computer skills including Microsoft Suites, Excel, data entry, and documentation.

  • Ability to work in a fast-paced and high-acuity nursing environment.

Additional Skills & Qualifications

  • OASIS EMR experience.

  • Excellent customer service and patient care skills.

  • Triage experience.

  • Experience with medical records management.

Work Environment

This is a hybrid opportunity with work hours from Monday to Friday, 8am to 5pm. Candidates must have a genuine interest in case management work.

Pay and Benefits

The pay range for this position is $35.00 - $45.00
