The role you’ll contribute:
The Advanced Hepatology Clinical Coordinator works with a multidisciplinary team to provide individualized care to patients who need advanced Hepatology care. The advanced therapies may include, but are not limited to, organ transplantation and medical therapy. The Clinical Coordinator identifies patients at risk for suboptimal outcomes and proposes a proactive plan of care. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Supports complex patient care issues, identifies patients at risk for suboptimal outcomes and proposes a proactive plan of care. Coordinates and/or supports patient education programs and evaluates impact of patient education programs.
Monitors practice standards and evaluates impact on patient care with identification of problematic systems/process issues. Assists with plans’ development to improve and monitor outcomes.
Monitors and evaluates nursing practice with identification of professional growth needs and utilizes. Knowledge about current research and evidence-based practices in area of expertise and recommends changes in practice as appropriate.
Acts as a liaison with the Hepatology patient population, the Liver Transplant Coordinator, and community care providers.
Advocates for Hepatology patients as an active member of the Hepatology team, which includes advanced assessment, planning and coordination of care, modification of the treatment plan as indicated, and the monitoring and evaluation of outcomes.
Assists and guides patients with non-medical needs as related to care insurance/referral process; pharmaceutical form and prescriptions and; disability/FMLA forms.
Documents per AH policy and standards.
Qualifications
The expertise and experiences you'll need to succeed:
EDUCATION AND EXPERIENCE REQUIRED :
- Three years nursing experience in inpatient and/or outpatient
EDUCATION AND EXPERIENCE PREFERRED :
BSN or Bachelor’s degree in a health-related field
Experience with participation in the care of patients with liver/GI disease, organ failure or chronic illness
Teaching/preceptor experience
Patient care management in the outpatient setting experience
