US Tech Solutions Personal Care Coordinator in Orange, California
The Personal Care Coordinator will support Whole Child Model (WCM) procedures. The incumbent will support with processing routine member referrals to Orange County Health Care Agency (OCHCA) for California Children Services (CCS) eligible conditions along with processing Annual Medical Review (AMR) and Inter-County Transfers (ICT) for WCM members.
The incumbent will work with health networks to obtain necessary clinical information and coordinate with OCHCA CCS department to support the referral submissions and processing the AMRs and ICTs.
Additionally, the incumbent will work with the members, family and other members of the health care team to complete a Health Needs Assessment (HNA) or Health Risk Assessment (HRA). The incumbent will ensure communication of the member’s HNA or HRA and care plan with the member, health network and health care team. The incumbent will identify barriers to members’ care and develop and make recommendations for comprehensive care plans at all levels of care.
Minimum Qualifications:
High School diploma or equivalent required.
1 year of experience working with the needs of pediatric members in a member service capacity required.
An equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may also be qualifying.
Preferred Qualifications:
Bachelor’s degree in health care management, social work, human services or related field.
Health Maintenance Organization (HMO), Medi-Cal and Pediatric Health services experience.
Bilingual in English and one of defined threshold languages (Arabic, Chinese, Farsi, Korean, Spanish, Vietnamese)
Knowledge & Abilities:
Develop rapport and establish and maintain effective working relationships with leadership and staff and external contacts at all levels and with diverse backgrounds.
Work independently and exercise sound judgment.
Communicate clearly and concisely, both orally and in writing.
Work a flexible schedule; available to participate in evening and weekend events.
Organize, be analytical, problem-solve and possess project management skills. Work in a fast-paced environment and in an efficient manner.
Manage multiple projects and identify opportunities for internal and external collaboration. Motivate and lead multi-program teams and external committees/coalitions.
Utilize computer and appropriate software (e.g., Microsoft Office: Word, Outlook, Excel, PowerPoint) and job specific applications/systems to produce correspondence, charts, spreadsheets, and/or other information applicable to the position assignment.
